Hospital ‐acquired anemia due to diagnostic and therapy‐related blood loss in inpatients with myasthenia gravis receiving therapeutic plasma exchange
ConclusionApproximately 50% of MG patients admitted for TPE developed hospital‐acquired anemia, which was directly correlated with the volume of blood collected for laboratory tests. A variety of strategies to reduce DLT could circumvent this iatrogenic complication. (Source: Journal of Clinical Apheresis)

Poor peripheral blood stem cell mobilization affects long ‐term outcomes in multiple myeloma patients undergoing autologous stem cell transplantation
DiscussionThese data indicate that poor mobilization can be predictable and is associated with more aggressive disease biology and worse outcomes, warranting intensive post‐ASCT management. (Source: Journal of Clinical Apheresis)

Successful treatment of pure red cell aplasia because of ABO major mismatched stem cell transplant
ConclusionThis demonstrates a case of PRCA caused by an ABO major mismatched allogeneic hematopoietic stem cell transplant that was potentially complicated by a parvovirus infection, which was treated with multiple therapeutic interventions including a series of therapeutic plasma exchanges, rituximab, and bortezomib. Successful resolution of the patient's PRCA was achieved. (Source: Journal of Clinical Apheresis)

Adsorptive granulocyte/monocyte apheresis use in severe ulcerative colitis and determination of changes in plasma cytokines
We report the cases of 3 patients with chronically active severe UC who underwent GMA due to an inadequate response to standard and rescue therapy, as well as changes to their plasma cytokine profile. All the patients presented severe UC that was only partially responsive to various immunosuppressive drugs, and they were, therefore, referred for colectomy; however, all 3 refused this option, which led to the compassionate use of GMA as a last therapeutic resort. Following GMA treatment, rapid normalization of the clinical, endoscopic and laboratory parameters was observed in all the patients. Despite having achieved a good...

An extremely rare splice site mutation in the gene encoding complement factor I in a patient with atypical hemolytic uremic syndrome
ConclusionAn extremely rare, heterozygous mutation in the gene encoding CFI likely affecting splicing was associated for the first time with aHUS. Sequencing was critical for rapid diagnosis and subsequent timely treatment with eculizumab, which resulted in improved renal function. (Source: Journal of Clinical Apheresis)

A transition from using multi ‐step procedures to a fully integrated system for performing extracorporeal photopheresis: A comparison of costs and efficiencies
ConclusionsFor hospitals considering a transition from multi‐step procedures to fully integrated methods for ECP where cost may be a barrier, time‐driven activity‐based costing should be utilized to gain a more comprehensive understanding the full benefit that such a transition offers. The example from our department confirmed that there were not just cost and time savings, but that the time efficiencies gained with CELLEX® allow for more patient treatments per year. Iatrogenic thyrotoxicosis and the role of therapeutic plasma exchange
We present a case of excessive thyroid replacement hormone ingestion treated by TPE. The patient presented with the clinical picture of thyroid storm, including cardiovascular compromise and massively elevated total and free T3 (525 ng/dL, nl 80–200 ng/dL and 28 pg/mL, nl 2.0–3.5 11 pg/mL), which failed medical therapy. A single, one plasma volume TPE was performed. Both total and free T3 demonstrated substantial declines immediately after TPE with the patient's mental status returning to near‐normal. Thyroid hormone extraction efficiency and collection efficacy were calculated as 37.1% and 40.8%, respectively. Prior...

Extracorporeal photopheresis in pediatric patients: Practical and technical considerations
Abstract In adults, extracorporeal photopheresis (ECP) is widely utilized for a variety of indications, most commonly cutaneous T‐cell lymphoma, acute or chronic graft‐versus‐host disease (GVHD), solid organ transplant rejection, and other autoimmune and T‐cell‐mediated disorders. In pediatric patients, the majority of case series and reports have focused on its use in the management of acute and chronic GVHD. Currently utilized ECP technologies were designed for adult patients and there are several challenges in adapting these technologies for use in children. In our review, we focus on practical considerations ...

Removal of focal segmental glomerulosclerosis (FSGS) factor suPAR using CytoSorb
Treatment of primary focal segmental glomerulosclerosis (FSGS) and its recurrence after kidney transplantation associated with rapid deterioration of kidney function remains to be challenging despite advances in immunosuppressive therapy. The presence of circulating factors has been postulated to be a pivotal player in the pathogenesis of FSGS, although suPAR and CLCF‐1 have been identified as the most promising causative factors. The potential therapeutic effect of suPAR elimination in an FSGS patient using CytoSorb, a hemoadsorption device that gained attention in the cytokine elimination in septic patients, was studie...
