A rare case of isolated trapezium fracture in motorcycle polytrauma patient in emergency department
Publication date: Available online 2 April 2016 Source:Journal of Acute Disease Author(s): Giancarlo De Nigris, Gabriele Falzarano, Antonio Medici, Steven James Mccabe, Grubor Predrag, Maria Liberata Meccariello, Luigi Meccariello Medical staff hardly recognize fractures of trapezium due to the small size and the complex anatomy of the wrist and the hand. Seventy percent of all carpal fractures are scaphoid’s fractures. Only 0.4% of all carpal injuries are trapezial fractures. In this paper, we presented a case of a high speed trauma where there were only trapezial fractures. Origin, diagnosis, differen...

A case of stroke during cardiac catheterisation: It's not common, but it is a double whammy!
We report a case of a 57-year-old lady with acute myocardial infarction who developed stroke on the cardiac catheterisation table. The case illustrates the need for a protocol of care, vigilant cardiologist and staff who can recognise the symptoms and signs as well as good collaborative efforts between the cardiology, neurology and, at times, the interventional radiology teams. Acute patho-toxicological indicators of methaemoglobinemia
Publication date: Available online 2 April 2016 Source:Journal of Acute Disease Author(s): Paul Chidoka Chikezie, Charles Uche Ekechukwu Methaemoglobin is formed when the haem iron of deoxyhaemoglobin is oxidized from its ferrous (Fe2+) to the ferric state (Fe3+) resulting in a haemoglobin molecule that is structurally and functionally altered, which leads to tissue hypoxia and metabolic acidosis. Classifications of methaemoglobinemia are based on clinical history, causative agents, pattern of transmission and optical spectrum presentation of blood specimen and level of erythrocyte NADH-methaemoglobin reductase act...

Carotid artery blowout producing massive hematemesis in the emergency department
Publication date: 1 March 2016 Source:Journal of Acute Disease, Volume 5, Issue 2 Author(s): Harrison K. Borno, Richard J. Menendez, John C. Chaloupka, Michael T. Dalley, David A. Farcy Carotid blowout syndrome (CBS) is a rare and fatal complication which arises from patients who have been treated for head and neck cancer. The incidence of CBS is rare and not commonly seen by emergency physicians. We review a case of a 68-year-old woman with a history of laryngectomy and chemo-radiation therapy presenting with massive oral bleeding and hypotension. Her course and treatments are highlighted, literature referr...

Pathological research on acute hepatic and renal tissue damage in Wistar rats induced by cocoa
Conclusions The pattern of alanine aminotransferase activity being more active than aspartate aminotransferase one in serum appeared to correlate with the extent of disarrangement of hepatic tissue architecture. The experimental rat groups exhibited no hyperbilirubinemia. Also, diets containing processed cocoa bean and raw cocoa bean products did not substantially interfere with the capacity of the hepatocytes to biosynthesize plasma proteins and the functionality of renal tissues. Evolving brain lesions in the follow-up CT scans 12 h after traumatic brain injury
Conclusions In case where the initial CT scan performed within 4 h of significant head injury was not correlated with the patient's neurology, it should be repeated within 12 h. (Source: Journal of Acute Disease)

Ventriculostomy related infection in intensive care unit: Diagnostic criteria and related conditions
Conclusions CSF routine cultures and biochemical studies are not recommended to diagnose VRI. Clinical signs, external ventricular drain manipulation and a drainage insertion over a week justify the routine measurement of proteinorrachia, glycorrhachia and the ratio of glycorrachia/glycemia. Displaced 3- and 4-part proximal humeral fractures: Evaluation and management with an intramedullary nail within 48 h, in the emergency department
Conclusions The nail utilized is provided with locking and multiplanar proximal screws and could be applied through a mini-invasive anterolateral approach. This enablesthe reduction of the fracture fragments, while preserving vascularization of the scapulo-humeral joint. Our results confirm that the indication of endomedullary nail could be extended to the treatment of complex proximal humeral fractures with 3 and 4 fragments (level of evidence IV). Research on relation of mortality and hemodynamics in patients with an acute pelvic ring fracture
