Cortical laminar necrosis due to refractory status epilepticus in a kidney transplant patient with cryptococcal meningitis
Publication date: Available online 17 November 2015 Source:Indian Journal of Transplantation Author(s): Zaheer Virani, Prashant Rajput, Pankaj Agarwal, Rashmi Badhe A 56-year-old diabetic male underwent kidney transplant in 2010. He was brought to the hospital with complaints of vomiting and altered sensorium of 10 days duration. Lumbar puncture revealed cryptococcal meningitis. He was promptly initiated on liposomal amphotericin B and flucytosine. Immunosuppressive agents tacrolimus and mycophenolate mofetil were discontinued. There was an initial improvement in his sensorium but a few days later, he developed...

Role of ficolin-3 in acute kidney graft rejection: A new diagnostic tool
Publication date: Available online 12 November 2015 Source:Indian Journal of Transplantation Author(s): Fateme Shamekhi Amiri The complement system has been implicated in a variety of conditions: autoimmune diseases, sepsis, transplantation, ischemia-reperfusion injuries, traumatic brain injury, infections, and bone biology. Complement activation in kidney transplantation may also induce allograft injury and contribute to delayed graft function. Activation of the complement system leads to the formation of molecules with proinflammatory properties. This may result in the killing of microorganisms, or it may lead to a...

Experience of hematopoietic stem cell transplantation (HSCT) in the patients infected with either hepatitis B or hepatitis C virus
Conclusions HSCT is a rugged job in Hbs-Ag positive or HCV-Ab positive patients due to more complications in the form of more neutropenia, GVHD, drug toxicity of chemotherapy, acute fulminant liver failure, fear of delaying of engraftment, and TRM. Careful evaluation before embarking on HSCT and intensive assessment against complications are warranted in Hbs-Ag positive and HCV-Ab positive recipients. (Source: Indian Journal of Transplantation)

Matched sibling allogenic transplant for an adult case of leukocyte adhesion deficiency (LAD-1)
We report first successful allogenic hematopoetic stem cell transplant of adult male with leukocyte adhesion deficiency. This patient was symptomatic for recurrent pyoderma gangrenosum since 10 years. He had received local treatments, systemic antibiotics, steroids, and cyclosporin but pyoderma was refractory leading to delay in hematopoetic stem cell transplant. The graft source was peripheral blood from 6/6 matched sibling. The conditioning regimen used was Busulphan, Cyclophosphamide, and antithymocyte globulin. Neutrophil engraftment occurred on day +14 and platelet engraftment occurred on day +24. Chimerism study of d...

Neutrophil-gelatinase associated lipocalin (N-GAL) to assess perioperative acute kidney injury in hand-assisted laparoscopic donor nephrectomy: A pilot study
Conclusion Significant AKI occurs with live donor nephrectomy. This is difficult to quantify using standard biochemistry due to the overwhelming effect of nephrectomy on eGFR. AKI is associated with poorer donor eGFR at 6 weeks. Pre-operative hypovolaemia plays a role in AKI. Optimisation of perioperative fluid management is likely to have a protective role. Anesthetic management of live donor renal transplantation in a recipient with Wegener's granulomatosis
Publication date: Available online 12 November 2015 Source:Indian Journal of Transplantation Author(s): Tanmay Tiwari, Divya Srivastava, Sandeep Sahu, Sanjay Dhiraaj Antineutrophil cytoplasmic autoantibody-associated vasculitis (AAV) is a common cause of rapidly progressive glomerulonephritis resulting in end-stage renal disease (ESRD). Wegener's granulamotosis (WG) is a rare disease of this group that have granulomatosis with polyangiitis, characterized by a triad of necrotizing granulomas in the upper and lower respiratory tracts, small vessel vasculitis, and glomerulonephritis. This case report illustrates t...

Uncommon presentation of tuberculosis in a renal allograft recipient
We describe a 55-year-old female patient who after 5 years of live donor renal transplant with stable immunosuppression and graft function presented with a chest swelling and a destructive bony lytic lesion diagnosed as sternal TB with radiological and microbiological investigations. She was successfully treated with antitubercular therapy to which she spontaneously responded. This case, to the best of our knowledge, is the first reported case of sternal TB in a renal allograft recipient to be successfully managed with antitubercular therapy. Spontaneous clearance of HCV infection in a renal allograft recipient
We report a case of a 43-year-old renal transplant recipient who had spontaneous clearance of HCV infection with reduction of immunosuppression to minimum without antiviral therapy. To the best of our knowledge, this is the first reported case in Indian literature. (Source: Indian Journal of Transplantation)
