Bone and metabolic complications of urinary diversions
Publication date: February 2015 Source:Endocrinología y Nutrición (English Edition), Volume 62, Issue 2 Author(s): Marta Cano Megías , Eva Golmayo Muñoz Delgado Hyperchloremic metabolic acidosis is a complication of urinary diversion using ileum or colon. Its prevalence ranges from 25% to 46% depending on the procedure used and renal function of the patient. It is a consequence of intestinal fluid and electrolyte exchange between intestinal mucosa and urine. The main mechanism is absorption of ammonium and chloride from urine. Long-term chronic metabolic acidosis in these patients may lead to impaired bone metabo...

Insulin resistance, β-cell dysfunction and differences in curves of plasma glucose and insulin in the intermediate points of the standard glucose tolerance test in adults with cystic fibrosis
Conclusions In over half the cases, peak blood glucose levels during a standard OGTT are reached in the intermediate time points, rather than at the usual time of 120min. Patients with cystic fibrosis and impaired glucose metabolism have a delayed insulin secretion during the standard OGTT due to loss of first-phase insulin secretion, with no differences in total insulin production. Absence of significant changes in HOMA-IR suggests that β-cell dysfunction is the main pathogenetic mechanism. (Source: Endocrinologia y Nutricion)

Cardiovascular risk stratification in overweight or obese patients in primary prevention. Implications for use of statins
Conclusions Risk stratification and use of statins varied in the overweight/obese population depending on the function used. Understanding of the relationship between scores and presence of CAP may optimize risk estimate. (Source: Endocrinologia y Nutricion)

Endocrine changes in histiocytosis of the hypothalamic–pituitary axis
This study analyzed the frequency and progression of pituitary hormone deficiencies and the radiographic findings in 9 patients (7 with LCH and 2 with ECD) with hypothalamic–pituitary (HP) axis. Eighty-nine percent of patients had DI (62% at diagnosis), and 78% had one or more anterior pituitary deficiencies (71% at diagnosis). HP involvement is relatively common in patients diagnosed with histiocytosis and hormone deficiencies may be present at diagnosis or appear gradually during the course of disease. Regular monitoring of these patients is recommended. (Source: Endocrinologia y Nutricion)

Implementation of subcutaneous insulin protocol for non-critically ill hospitalized patients in Andalusian tertiary care hospitals
Conclusions Strategies are needed to improve implementation of the inpatient subcutaneous insulin protocol, particularly in surgical departments. Sliding scale insulin is still the most common alternative to insulin regimen based on basal-bolus-correction scheduled insulin. Metabolic control assessment during hospitalization should be encouraged. (Source: Endocrinologia y Nutricion)

Determination of thyrotropin reference values in an adult Mexican population
Conclusions The TSH reference interval in the reference population was 0.71 (CI 0.65–0.77) to 4.88mIU/L (CI 4.5–5.3); these limits may be influenced by iodine nutritional status in this population. (Source: Endocrinologia y Nutricion)

Preoperative TSH level and risk of thyroid cancer in patients with nodular thyroid disease: nodule size contribution
Conclusions Our study supported an association between preoperative TSH levels and risk of DTC in patients with NTD. There was also a direct relationship between malignant nodule size and TSH levels. By contrast, no relationship was found between the size of benign nodules and TSH levels. (Source: Endocrinologia y Nutricion)
