Effectiveness of enhanced pulse oximetry sonifications for conveying oxygen saturation ranges: a laboratory comparison of five auditory displays
ConclusionsEnhanced sonifications are more informative than conventional sonification. The implication is that they might allow anaesthetists to judge better when desaturation decreases below, or returns to, a target range. (Source: British Journal of Anaesthesia)

Activation of cannabinoid receptor 1 is involved in protection against mitochondrial dysfunction and cerebral ischaemic tolerance induced by isoflurane preconditioning
Conclusions. Our results indicate that IPC elicits brain ischaemic tolerance and mitochondrial protection by activating the CB1R, which provides a new mechanism for IPC-induced neuroprotection against cerebral ischaemia. (Source: British Journal of Anaesthesia)

Is postspinal hypotension a sign of impaired cardiac performance in the elderly? An observational mechanistic study
Conclusions. A decrease in cardiac output from baseline before spinal anaesthesia and an inability to increase it after induction may be important features of postspinal hypotension in elderly patients. (Source: British Journal of Anaesthesia)

Changes in platelet Bax levels contribute to impaired platelet response to thrombin after cardiopulmonary bypass: prospective observational clinical and laboratory investigations
Conclusions. CPB increases platelet Bax expression, which contributes to reduced platelet-surface GPIb expression and thrombin-induced platelet calcium changes. These changes in platelet apoptotic signalling might contribute to platelet dysfunction after CPB.Clinical trial registration. UMIN Clinical Trials Registry (number UMIN000006033). Overuse of preoperative laboratory coagulation testing and ABO blood typing: a French national study
ConclusionsStandard laboratory coagulation tests and ABO blood typing are still routinely prescribed before surgery and anaesthesia despite current guidelines. This over-prescription represents a high and unnecessary cost, and should therefore be addressed. (Source: British Journal of Anaesthesia)

Intraoperative oliguria predicts acute kidney injury after major abdominal surgery
Conclusions. Among patients undergoing major abdominal surgery, intraoperative oliguria<0.3  ml kg−1 h−1 was significantly associated with increased risk of postoperative AKI. (Source: British Journal of Anaesthesia)

Death or near-death in patients with obstructive sleep apnoea: a compendium of case reports of critical complications
AbstractThe care of surgical patients with obstructive sleep apnoea (OSA) invokes concerns with safety and liability because of the risk that exists for perioperative death or near-death. The purpose of this review is to analyse the available literature to identify risk factors for perioperative critical complications in patients with OSA. Literature reports were screened for life threatening complications and deaths in surgical patients with OSA. The critical complications were sub-grouped as death/near-death events (death and anoxic brain damage)vs critical respiratory events (CRE)/other events and analysed for various r...

Gastric emptying for liquids of different compositions in children
ConclusionsDespite early differences, clearance from the stomach of apple juice, 2% milk or Ensure Clear is similar at the terminal phase, which is the period of greatest relevance to preoperative fasting recommendations. The stomach is essentially clear by 3-3.5 h for all three drinks studied. The differentiation between liquids in current guidelines is not supported by this study.Clinical trial registrationclinicaltrials.gov NCT02938065clinicaltrials.gov/ct2/show/NCT02938065. Novel method for intraoperative assessment of cerebral autoregulation by paced breathing
Conclusions. During surgery, cerebral autoregulation indices were similar to values determined before surgery. This indicates that CA can be quantified reliably and non-invasively using this novel method and confirms earlier evidence that CA is unaffected by sevoflurane anaesthesia.Clinical trial registration. Effectiveness of tip rotation in fibreoptic bronchoscopy under different experimental conditions: an in vitro crossover study
Conclusions.‘Cord’ turning of the fibrescope appears to be more effective in rotating the tip than a turn of the fibrescope ‘body’ only. Straightening the fibrescope cord and combined ‘body’ and ‘cord’ turning are recommended. (Source: British Journal of Anaesthesia)

An experimental study comparing the respiratory effects of tapentadol and oxycodone in healthy volunteers
ConclusionsIn this exploratory study we observed that both tapentadol and oxycodone produce respiratory depression. Tapentadol 100  mg but not 150 mg had a modest respiratory advantage over oxycodone 20 mg. Further studies are needed to explore how these results translate to the clinical setting.
