Pallidal and thalamic neural oscillatory patterns in tourette's syndrome
The objective of this study was to demonstrate a pathophysiological association of pallidal and thalamic local field potentials (LFPs) with TS.MethodsNine medication ‐refractory TS patients were included in the study. Intracerebral LFPs were recorded simultaneously from bilateral pallidal and thalamic DBS electrodes. Spectral and temporal dynamics of pallidal and thalamic oscillations were characterized and correlated with preoperative Yale Global Tic Severity Scale (YGTSS) scores.ResultsPeaks of activity in the theta (3 –12Hz) and beta (13–35Hz) were present in pallidal and thalamic recordings from all patients (3 w...

Randomized double ‐blinded clinical trial of 5% dextrose versus triamcinolone injection for carpal tunnel syndrome patients
ObjectivePerineural injection with 5% dextrose (D5W) is a novel strategy in the treatment of carpal tunnel syndrome (CTS). In contrast, perineural injection with corticosteroid has been used for decades for treating CTS, but possible neurotoxicity has been a major concern. No studies investigating the comparative effects have been published so far. The authors performed a prospective, randomized, double ‐blinded, head‐to‐head comparative trial to compare these two approaches for patients having mild‐to‐moderate CTS.MethodsFifty ‐four participants with mild‐to‐moderate CTS were randomly divided into dextrose...

Low ‐voltage fast seizures in humans begin with increased interneuron firing
ObjectiveIntracellular recordings from cells in entorhinal cortex tissue slices show that low ‐voltage fast (LVF) onset seizures are generated by inhibitory events. Here, we determined whether increased firing of interneurons occurs at the onset of spontaneous mesial–temporal LVF seizures recorded in patients.MethodsThe seizure onset zone (SOZ) was identified using visual inspection of the intracranial electroencephalogram. We used wavelet clustering and temporal autocorrelations to characterize changes in single ‐unit activity during the onset of LVF seizures recorded from microelectrodes in mesial–temporal struct...

Association between carotid atheroma and cerebral cortex structure at age 73 years
ObjectiveTo examine the relationship between carotid atherosclerosis and cerebral cortical thickness and investigate whether cortical thickness mediates the association between carotid atheroma and relative cognitive decline.MethodsWe assessed 554 community ‐dwelling subjects (male/female: 296/258) from the Lothian Birth Cohort 1936 who underwent brain magnetic resonance imaging and carotid Doppler ultrasound studies at age 73 years. The relationship between carotid atherosclerosis markers (internal carotid artery stenosis, intima–media thickness, velocity, pulsatility, and resistivity indexes) and vertex‐wide cerebr...

Scalp electroencephalographic biomarkers in epilepsy patients with focal cortical dysplasia
This study confirms that there are several surface EEG biomarkers significantly associated with an underlying cortical dysplasia. These biomarkers may aid in localizing suspicious brain regions and provide evidence for dysplastic brain tissue also in nonlesional patients of either histological FCD subtype. Lamotrigine clearance increases by 5 weeks gestational age: Relationship to estradiol concentrations and gestational age
ObjectiveTo determine how early lamotrigine clearance (LTG ‐CL/F) increases during early pregnancy in women with epilepsy and to quantify the relationship of LTG‐CL/F to estradiol concentrations and gestational week.MethodsThis was a multicenter, observational study of pregnant women with epilepsy on lamotrigine and no interacting concomitant medications, employing frequent blood sampling prior to and early in pregnancy. A population mixed ‐effects modeling approach was used to describe the relationship between LTG‐CL/F and gestational week and between LTG‐CL/F and estradiol. Akaike information criterion (AIC) co...

In vivo characterization of white matter pathology in premanifest huntington's disease
ObjectiveHuntington's disease (HD) is a monogenic, fully penetrant neurodegenerative disorder, providing an ideal model for understanding brain changes occurring in the years prior to disease onset. Diffusion tensor imaging (DTI) studies show widespread white matter disorganization in the early premanifest stages (pre ‐HD). However, although DTI has proved informative, it provides only limited information about underlying changes in tissue properties. Neurite orientation dispersion and density imaging (NODDI) is a novel magnetic resonance imaging (MRI) technique for characterizing axonal pathology more specific ally, pro...

Variation at the TRIM11 locus modifies progressive supranuclear palsy phenotype
ObjectiveThe basis for clinical variation related to underlying progressive supranuclear palsy (PSP) pathology is unknown. We performed a genome ‐wide association study (GWAS) to identify genetic determinants of PSP phenotype.MethodsTwo independent pathological and clinically diagnosed PSP cohorts were genotyped and phenotyped to create Richardson syndrome (RS) and non ‐RS groups. We carried out separate logistic regression GWASs to compare RS and non‐RS groups and then combined datasets to carry out a whole cohort analysis (RS = 367, non‐RS = 130). We validated our findings in a third cohort by referring t...
