Atheromatous plaque formation in a man with high-risk atherosclerosis after Gamma Knife stereotactic radiosurgery for trigeminal neuralgia
We report a rare case of atheromatous plaque formation in the offending parent artery of a 39-year-old man 11 months after Gamma Knife stereotactic radiosurgery (GKSR) for trigeminal neuralgia (TN). Focal atheromatous changes to the parent vessel remote from the root entry zone (REZ) of the trigeminal nerve were discovered during rescue open surgery; this has seldom been reported. Our report suggests that younger male TN patients with hyperlipidemia who receive GKSR may have an increased risk of post-radiation atheromatous formation. A review of the literature is carried out together with a discussion of the possible mecha...

Composite type A thymoma and diffuse large B-cell lymphoma
We describe a 74-year-old man who presented with general weakness, neck lymphadenopathy, night sweats, and body weight loss. A right anterior mediastinal mass was found on computed tomography of the chest. The immunohistochemical stains AE1/AE3, CD20, CD3, and MUM-1 confirmed the different components of the mediastinal tumor. A heavy-chain gene clonality assay and light-chain gene clonality assay confirmed the B-cell clonality of the mediastinal tumor and neck lymph node. The patient had received a complete course of chemotherapy, and the result of positron emission tomography–computed tomography showed complete remissio...

Refractory hypocalcemia and hypomagnesemia associated with the use of an oral proton-pump inhibitor in a patient with hypoparathyroidism
We report a 64-year-old man with a malignant neoplasm of the hypopharynx. After a total pharyngolaryngectomy, bilateral selective neck dissection (levels II, III, and IV), and right thyroidectomy, he developed hypocalcemia, hypomagnesemia, and hypoparathyroidism. After the administration of calcium and magnesium supplements, the patient's serum calcium and magnesium levels were still abnormal. The patient was later found to be taking an oral proton-pump inhibitor (esomeprazole) to treat a peptic ulcer. After discontinuing the oral proton-pump inhibitor, the patient was found to have normal serum calcium and magnesium level...

Development of an advance care planning booklet in Taiwan
Conclusion An ACP booklet facilitates discussion and decision-making related to end-of-life care. Furthermore, the present findings indicated that, when carrying out an ACP discussion, not only are the preferences and values of people important, but also factors related to their culture. Predictors of lack of improvement in the left ventricular ejection fraction in patients with congestive heart failure 18 months after commencement of a disease management program: A prospective observational study
Conclusion Younger age, no schooling, not being married, and elevated triglycerides at baseline emerged as significant and independent predictors of a lack of improvement in the LVEF after 18 months of disease management intervention. These findings can serve as a basis for resource allocation when planning future disease management programs. (Source: Tzu Chi Medical Journal)

Effects of denosumab on bone mineral density and renal function in postmenopausal women transitioning from raloxifene
Conclusion Patients who transitioned from raloxifene to denosumab showed good adherence with treatment and significant improvement in BMD in the spine and hip without a negative influence on renal function. Discussing the characteristics of both drugs and the best antiosteoporotic treatment with patients is critical for satisfactory results in the prevention of fractures. Three deduced probable human-leukocyte-antigen haplotypes associated with HLA-DQB1*03:26 and -DRB1*14:141 from Taiwanese unrelated bone-marrow hematopoietic-stem-cell donors: Two case analyses
Conclusion Information on the ethnicity of the DQB1*03:26 and DRB1*14:141 alleles, and the deduced probable HLA haplotypes associated with the low-incidence alleles that we report here is of value to HLA testing laboratories for reference purposes. In addition, they can be used by stem-cell-transplantation-donor-search coordinators in order to determine a strategy for finding compatible donors in unrelated bone-marrow-donor registries when patients carry these uncommon HLA alleles. Effects of subthalamic nucleus deep brain stimulation on quality of life and motor and depressive symptoms in Parkinson's disease
The objective of this paper is to review the available literature concerning the effects of subthalamic nucleus deep brain stimulation (STN-DBS) on quality of life (QoL) and motor and depressive symptoms in patients with Parkinson's disease (PD). These studies demonstrate that STN-DBS has an effect on QoL and symptoms in patients with PD. There was a significant improvement in QoL following STN-DBS compared with no improvement after medical therapy. PD patients treated with STN-DBS had a 40–50% improvement in motor function. Nevertheless, depressive symptoms did not reveal consistent change. (Source: Tzu Chi Medical Journal)

Pathophysiology of interstitial cystitis/bladder pain syndrome
Publication date: December 2015 Source:Tzu Chi Medical Journal, Volume 27, Issue 4 Author(s): Qian-Shen Ke, Hann-Chorng Kuo Interstitial cystitis (IC) is a heterogeneous syndrome characterized by bladder pain and is associated with frequency and nocturia. Research findings have proposed several pathophysiological mechanisms including epithelial dysfunction, activation of mast cells, neurogenic inflammation, autoimmunity, and occult infection. One of the most common findings in IC bladders is denudation or thinning of the bladder epithelium, suggesting an altered regulation of urothelial homeostasis. Different pheno...

Practical points in the medical treatment of overactive bladder and nocturia in the elderly
Publication date: Available online 25 January 2016 Source:Tzu Chi Medical Journal Author(s): Jing-Liang Chen, Sheng-Fu Chen, Yuan-Hong Jiang, Hann-Chorng Kuo The prevalence of overactive bladder (OAB) increases with age. Degeneration of the central nervous system in the elderly has been proposed as one of the pathogenic factors of OAB. Antimuscarinic therapy is effective in the treatment of OAB; however, intolerable systemic adverse events and cognitive dysfunction during treatment with nonselective antimuscarinic agents is of growing concern in elderly patients. The newly developed beta-3 adrenoceptor agonist ...
