A suction pipe segment as an alternative to a tube stop when carrying out tracheal intubation with a Trachway intubating stylet with a longer shaft
Publication date: September 2015 Source:Tzu Chi Medical Journal, Volume 27, Issue 3 Author(s): Pei-Jen Kuo, Yu-Chi Lin, Ming-Hwang Shyr The Trachway video intubating (TVI) stylet has been widely investigated and is used for both normal and difficult intubations. Practitioners may use the longer 420 mm shaft stylet for orotracheal intubation. However, an endotracheal tube stop is needed to prevent displacement of the endotracheal tube during stylet advancement in the oral cavity. The original tube stop, which can be purchased from the manufacturer, is small and easy to lose. We designed a device derived from a 95...

Orbital sarcoidosis
We report a 50-year-old woman with orbital sarcoidosis. The initial presentation was painless swelling in the upper eyelid and mildly limited extraocular motion. A heterogeneous mass with a surrounding bony defect was noted on an orbital computerized tomography scan. Histopathological examination revealed noncaseating granulomas, which were compatible with a diagnosis of sarcoidosis. Elevation of the serum angiotensin-converting enzyme level and mediastinal lymphadenopathy, found on chest computerized tomography, also confirmed the diagnosis. The patient was treated with low-dose corticosteroids for 5 weeks and showed a f...

Management of complex open abdominal wounds: Report of two cases
This report discusses the successful management of two complex open abdominal wounds. The wounds were temporarily closed with a silastic sheet. After several dressing changes with vacuum-assisted closure (VAC), bilateral bipedicle advancement flaps were used to close the complex open abdominal wound. Case 1: a 41-year-old man underwent gastric bypass surgery for morbid obesity. Wound dehiscence resulted from a sudden increase in intra-abdominal pressure while weaning from mechanical ventilation. The huge open wound was temporarily covered with a silastic sheet. After serial VAC dressing changes, the wound was progressively...

Extracranial–intracranial bypass in the treatment of complex or giant internal carotid artery aneurysms
Conclusions Cerebral revascularization plays an important role in the treatment of complex ICA aneurysms that have a significant mass effect on the optic nerve or require occlusion of the parent ICA as a salvage procedure. (Source: Tzu Chi Medical Journal)

Comparison of improvement in quality of life between continuous positive airway pressure and autotitrating positive airway pressure treatment for obstructive sleep apnea: A randomized crossover study
Conclusion In our study, the CPAP patients had a higher pressure and lower AHI than the APAP patients. OSA patients preferred APAP. Both CPAP and APAP can improve general health and vitality in OSA patients. (Source: Tzu Chi Medical Journal)

Two deduced probable HLA-A*24:287-associated HLA haplotypes (A*24:287-B*40-DRB1*15 and A*24:287-B*58-DRB1*03:01) found in Taiwanese unrelated hematopoietic bone marrow stem cell donors-case analysis
Conclusion Information on the deduced probable HLA haplotypes in association with the low incidence A*24:287 allele that we report here are of value for HLA testing laboratories for reference purposes. In addition, they can be used by stem cell transplantation donor search coordinators to determine a strategy for finding compatible donors in unrelated bone marrow donor registries when a patient has this uncommon HLA allele. One-lung circumvention, an interventional strategy for pulmonary salvage in acute paraquat poisoning: An evidence-based review
Publication date: September 2015 Source:Tzu Chi Medical Journal, Volume 27, Issue 3 Author(s): Sayed Mahdi Marashi, Hojatollah Raji, Zeynab Nasri-Nasrabadi, Mohammad Majidi, Maryam Vasheghani-Farahani, Abdollah Abbaspour, Ahmad Ghorbani, Shayesteh Vasigh Paraquat is highly toxic to humans. Peak plasma levels are reached within 1 hour after intentional ingestion of this poison, followed by a rapid decline because of its distribution to the extravascular compartment and renal elimination. It induces generation of free oxygen radicals and consumption of intracellular nicotinamide adenine dinucleotide phosp...

Neovascular glaucoma following Nd:YAG laser capsulotomy in a patient with diabetes and syphilitic uveitis
Publication date: Available online 5 September 2015 Source:Tzu Chi Medical Journal Author(s): Hong-Zin Lin, Yuan-Chieh Lee A diabetic woman had been suffering from progressively blurry vision in the right eye for 1 year after cataract surgery. Slit lamp examination revealed bilateral band keratopathy with quiescent anterior-chamber reaction, an intumescent cataract in the left eye, and dense posterior capsular opacity in the right eye. Capsulotomy was performed, but this led to neovascular glaucoma. Because of a refractory elevation in intraocular pressure, trabeculectomy with mitomycin C treatment, vitrectomy with...

Effects of obesity on the association between common variations in the TBX5 gene and matrix metalloproteinase 9 levels in Taiwanese
Conclusion TBX5 genotypes/haplotypes are independently associated with MMP9 in Taiwanese individuals and occur predominantly in nonobese people. These associations may broaden our understanding of the mechanism underlying T-box family gene activity and related cardiovascular pathologies. (Source: Tzu Chi Medical Journal)
