Predictive accuracy of risk scales following self-harm: multicentre, prospective cohort study [PAPERS]
Conclusions Risk scales following self-harm have limited clinical utility and may waste valuable resources. Most scales performed no better than clinician or patient ratings of risk. Some performed considerably worse. Positive predictive values were modest. In line with national guidelines, risk scales should not be used to determine patient management or predict self-harm. Quetiapine v. lithium in the maintenance phase following a first episode of mania: randomised controlled trial [PAPERS]
Conclusions In people with first-episode mania treated with a combination of lithium and quetiapine, continuation treatment with lithium rather than quetiapine is superior in terms of mean levels of symptoms during a 1-year evolution. (Source: The British Journal of Psychiatry)

Disruptive mood dysregulation disorder in offspring of parents with depression and bipolar disorder [PAPERS]
Conclusions Our results suggest that DMDD is not specifically associated with a family history of bipolar disorder and may be associated with parental depression. (Source: The British Journal of Psychiatry)

Strategies to prevent death by suicide: meta-analysis of randomised controlled trials [Review articles]
Conclusions The WHO BIC is a promising suicide prevention strategy. No other intervention showed a statistically significant effect in reducing suicide. (Source: The British Journal of Psychiatry)

Predicting suicidal behaviours using clinical instruments: systematic review and meta-analysis of positive predictive values for risk scales [Review articles]
Conclusions No ‘high-risk’ classification was clinically useful. Prevalence imposes a ceiling on PPV. Treatment should reduce exposure to modifiable risk factors and offer effective interventions for selected subpopulations and unselected clinical populations. Predictive properties of risk assessment instruments following self-harm [EDITORIALS]
This month's BJPsych publishes two important studies concerned with the use of risk assessment scales after self-harm, one a systematic review and the other a multicentre cohort study. We agree with the authors: that each study adds weight to the existing evidence that points towards avoiding the use of such scales in clinical practice. (Source: The British Journal of Psychiatry)
