Acceptability of a Nipple Shield Delivery System Administering Antiviral Agents to Prevent Mother-to-Child Transmission of HIV through Breastfeeding
Conclusion: The NSDS is a potentially acceptable method of PMTCT during breastfeeding. Further studies are needed to confirm acceptability, safety, and efficacy. For NSDS adoption to PMTCT, strategies will need to be developed to minimize HIV-related stigma and to ensure that continuous hygiene of the device is maintained. (Source: Journal of Human Lactation)

The Intricacies of Induced Lactation for Same-Sex Mothers of an Adopted Child
The definition of a modern family is changing. In this case study, we describe the breastfeeding experience of a child receiving human milk from all 3 of his mothers: his 2 adoptive mothers, who induced lactation to nurse him, and his birth mother, who shared in his early feeding during the open adoption process and continued to pump and send milk to him for several months. We review the lactation protocol used by his adoptive mothers and the unique difficulties inherent in this multi-mother family dynamic. Both adoptive mothers successfully induced moderate milk production using a combination of hormonal birth control, do...

Use of Domperidone as a Galactagogue Drug: A Systematic Review of the Benefit-Risk Ratio
The objective of this review was to assess the benefit-risk ratio of domperidone for stimulating lactation. The benefit-risk ratio of domperidone as a galactagogue was assessed following a literature search of the PubMed database up to July 2013. Four studies were selected to assess domperidone efficacy and demonstrated an increased milk production. The limited data (60 mother-baby pairs) and the moderate methodological quality of 1 study remain insufficient to conclude on domperidone efficacy. Regarding the safety of domperidone, 7 studies were selected that exposed 113 infants to domperidone through breastfeeding. No adv...

Making the First Days of Life Safer: Preventing Sudden Unexpected Postnatal Collapse while Promoting Breastfeeding
Early and prolonged skin-to-skin contact (SSC) after birth between a mother and her newborn has been shown to generate beneficial effects on the mother-infant relationship and breastfeeding. Close mother-infant body contact immediately after birth positively enhances exclusive breastfeeding during the hospital stay, with a dose-response relationship. Skin-to-skin contact may ease the infant’s transition to extra-uterine life and helps regulate the infant’s body temperature and nursing behavior. However, reports of sudden unexpected postnatal collapse (SUPC) soon after birth, in healthy term neonates, in associa...

Using a Wellness Program to Promote a Culture of Breastfeeding in the Workplace: Oregon Health & Science University's Experience
In the United States, many women stop breastfeeding within the first month that they return to work. Working mothers experience challenges in maintaining milk supply and finding the time and space to express breast milk or feed their babies in workplace settings. Changing attitudes and culture within the workplace may be accomplished in conjunction with ensuring compliance with state and federal laws regarding breastfeeding to improve breastfeeding rates after return to work. Employee wellness programs can be 1 avenue to promote breastfeeding and human milk donation as healthy behaviors. (Source: Journal of Human Lactation)

Working Mothers of the World Health Organization Western Pacific Offices: Lessons and Experiences to Protect, Promote, and Support Breastfeeding
Optimal breastfeeding saves lives. However, suboptimal breastfeeding is prevalent, primarily resulting from inappropriate promotion of infant formula and challenges of working mothers to continue breastfeeding. The article aims to determine the extent to which World Health Organization (WHO) policies protect, promote, and support breastfeeding women working at the WHO, Western Pacific Region. An online survey targeted all female WHO and contractual staff in all country and regional offices, who delivered a baby between July 24, 2008 and July 24, 2013.
