Publication ethics from the ground up
Summary It is relatively easy to begin policy documents with a general assertion that ethics will be followed. Less obvious is how to ensure that day‐to‐day activities are consonant with ethical standards. We suggest that using day‐to‐day publication activities as the driver for building policies and procedures can promote ethical practices from the ground up. Although basic principles of ethical publication practice may seem straightforward to some, for others this information may require explanation, interpretation and context. Effective policy development includes big‐picture items as well as more day‐to‐d...

The pattern of prescribing of glucose modulating agents for type 2 diabetes in general practices in England 2016/17
ConclusionsOur work demonstrates the significant cost of medication to modulate tissue glucose levels in type 2 diabetes and the dominance of some non‐generic preparations in terms of number of prescriptions and overall spend. There are some older sulphonylureas in use, which should not generally be prescribed. Regular audit of patient treatment at a general practice level will ensure appropriate targeted use of licensed medications and of their cost effectiveness. The effect of splenectomy on the levels of PCV ‐13‐induced memory B‐ and T cells
ConclusionOur results suggest that spleen is important, but not essential, for survival and/or generation of memory IgG B cells. In contrast, it seems to be indispensable for PCV‐13‐specific memory TH1‐cell levels. Studies enhancing the levels of vaccine‐induced memory cells and further enlightening the immune responses in asplenic individuals are required to develop more effective vaccination strategies against OPSI. Glycaemic control and treatment of type 2 diabetes in adults aged 75  years or older
ConclusionIntensive treatment among older adults with diabetes mellitus type 2 is common in primary care clinical practice in our area. Intensive glycaemic control confers an increased risk of hypoglycaemia and little benefit among older individuals with diabetes. Physicians should take care more not to harm those populations and treatment should be de‐intensified to reduce the risk of hypoglycaemia. (Source: International Journal of Clinical Practice)
