CONCLUSION: Survival analysis applied to primary care electronic medical data is a research approach that has been frequently used in recent times. CONCLUSION: Examples of best practice were found in the study hospitals but there were also areas for improvement. Coding managers would benefit from greater support in the form of increased opportunities for management training and a better method for calculating CC workforce numbers. A career pathway is needed for CCs to progress from entry to expert CC, mentor, manager and quality controller. Demographic characteristics of Australian health consumers who were early registrants for opt-in personally controlled electronic health records.

A structured review of chronic care model components supporting transition between healthcare service delivery types for older people with multiple chronic diseases.

CONCLUSION: Data entry errors may affect the amount of funding received by a healthcare organisation, which in turn may affect the quality of treatment provided to clients due to the possibility of underfunding the organisation.
