Review: Stent Fracture in the Drug-Eluting Stent Era
Stent fracture has been recognized as one cause of stent failure and has been associated with in-stent restenosis and stent thrombosis, even in 2nd-generation drug-eluting stents. Given the wide use of drug-eluting stents and paucity of contemporary data available concerning stent fracture, we reviewed clinical studies and the Food and Drug Administration's Manufacturer and User Device Experience (MAUDE) database to analyze the current trends, mechanisms, predictors, outcomes and treatment for stent fracture. Successful Deployment of Polytetrafluoroethylene-Covered Stent to Seal Left Internal Mammary Artery Graft Perforation Due to Guide Catheter Extension System
We report the successful deployment of a polytetrafluoroethylene (PTFE)-covered stent to seal the LIMA graft perforation due to the guide catheter extension system. Percutaneous coronary intervention was performed for a culprit lesion of the distal left circumflex via the LIMA graft. (Source: Cardiovascular Revascularization Medicine)

“Subintimal External Crush” Technique for a “Balloon Uncrossable” Chronic Total Occlusion
We describe a case of a “balloon uncrossable” CTO in which balloon crossing was achieved after subintimal dilation and “external crushing” of the CTO lesion resulting in significant modification of the CTO atheromatous plaque. (Source: Cardiovascular Revascularization Medicine)

Radial artery perforation treated with balloon tracking and guide catheter tamponade – a case series
We describe a series of 7 cases where this technique was used to both treat radial artery perforation and to complete the angioplasty via the radial approach. (Source: Cardiovascular Revascularization Medicine)

Combined use of export catheter and penumbra vacuum thromboaspiration in a challenging case of acute common carotid artery occlusion
A 76-year-old hypertensive man with previous bilateral iliac stenting was admitted in our center for acute stroke with an NIH score of 20 at 6  h from symptoms onset. The common carotid occlusion with a huge thrombus and a very calcified plaque has been successfully recanalyzed with a combination of coronary total occlusion technique, filter-aided coronary manual thrombectomy and Penumbra vacuum thrombectomy systems. Association of elevated triglycerides and acute myocardial infarction in young Hispanics
Previous studies have demonstrated that acute myocardial infarction (AMI) in young patients (age < 45years) is associated with a high prevalence of smoking, obesity, hyperlipidemia and single vessel coronary artery disease (CAD). Hispanics represent the largest growing ethnic minority in the United States, yet features of AMI in young Hispanics have not been described. (Source: Cardiovascular Revascularization Medicine)

Combined Use of Export Catheter and Penumbra Vacuum Thromboaspiration in a Challenging Case of Acute Common Carotid Artery Occlusion
A 76-year-old hypertensive man with previous bilateral iliac stenting was admitted in our center for acute stroke with an NIH score of 20 at 6h from symptoms onset. A Doppler ultrasonography scan revealed a thrombotic occlusion the right common carotid artery: plaque appearance was hypercalficic with a huge thrombus burden. A diffusion cerebral magnetic resonance imaging demonstrated a two focal hyperacute ischemic lesions in the right semioval region (Fig. 1, panel A, red stars). A Neuron Max 6F (Penumbra Inc., Alameda, CA, USA) long sheath was advanced through the femoral artery approach over a .035″ Supracor guide wir...

Incidence, feasibility and outcome of percutaneous coronary intervention after transcatheter aortic valve implantation with a self-expanding prosthesis. Results from a single center experience
Percutaneous coronary intervention (PCI) after transcatheter aortic valve implantation (TAVI) can become technically challenging after implantation of the self-expanding Medtronic CoreValve (MCV) device, which completely covers the aortic root. The aim of this study was to report on the incidence, feasibility and outcome of PCI after TAVI with the MCV device. (Source: Cardiovascular Revascularization Medicine)

Comparison of fractional flow reserve measurements using intracoronary adenosine versus intracoronary sodium nitroprusside infusions in moderately stenotic coronary artery lesions
The aim of this study was to investigate the efficacy and safety of intracoronary (IC) sodium nitroprusside infusion in comparison to IC adenosine for fractional flow reserve (FFR) measurement in moderately diseased coronary artery lesions for functional assessment. Importance of IVUS in the treatment with retrograde approach of a long in-stent chronic total occlusion of ostial right coronary artery: A case report
A 43year-old male, with a long history of coronary artery disease, was electively admitted to our institution because of dyspnea for moderate physical efforts (NYHA 2) related to an in-stent chronic total occlusion of ostial RCA and with demonstration of a significant area of inducible myocardial ischemia at stress echocardiography. After a gentle attempt of antegrade approach, the in-stent CTO of ostial RCA was successfully reopened with a retrograde approach using a knuckle technique. After IVUS evaluation which showed that the course of the retrograde guidewire was mostly within the stent strut circumference with the ex...
