Increased risk of mycotic infections associated with sodium –glucose co‐transporter 2 inhibitors: a prescription sequence symmetry analysis
ConclusionInitiation of SGLT2i was associated with an increased risk for mycotic infections. Findings from this commercially insured population in the real world are consistent with evidence available from clinical trials. (Source: British Journal of Clinical Pharmacology)

The journey of metformin from glycaemic control to mTOR inhibition and the suppression of tumour growth
Our knowledge of the effect of metformin on human health is increasing. In addition to its ability to improve the control of hyperglycaemia, metformin has been shown to reduce the burden o,f ageing via effects on damaged DNA and the process of apoptosis. Studies have shown that metformin may reduce the risk of cardiovascular disease through influences on body weight, blood pressure, cholesterol levels and the progression of atherosclerosis. Studies also suggest that metformin may be beneficial for neuro ‐psychiatric disorders, cognitive impairment and in reducing the risk of dementia, erectile dysfunction and Duchenne mu...

Paediatric Extrapolation: A Necessary Paradigm Shift
AbstractLegislative initiatives have been successful in increasing the availability of approved therapies for paediatric patients. However, additional measures to ensure the timely completion of paediatric studies are necessary to further increase the number of medicines available to children.Over the last three years, international experts convened to revise the ICH E11 guideline on clinical investigations of medicinal products in paediatric populations to harmonise approaches to paediatric extrapolation, striving to reduce substantial differences between regions in the acceptance of data for global paediatric medicine de...

Predictors of First ‐Year Nonadherence and Discontinuation of Statins among Older Adults: A Retrospective Cohort Study
ConclusionsAmong older adults prescribed statins, first ‐year nonadherence and discontinuation is high. Specific population subgroups such as people aged ≥85 years, those with diabetes or anxiety may require additional attention to improve statin adherence. Efficacy of DPP ‐4 inhibitors, GLP‐1 analogues, and SGLT2 inhibitors as add‐ons to metformin monotherapy in T2DM patients: A model‐based meta‐analysis (149/150)
ConclusionsOnce weekly exenatide, liraglutide, and dulaglutide provided better hypoglycaemic effects among the antidiabetic drugs analysed. Long ‐acting GLP‐1r appears to be more useful for T2DM patients inadequately controlled with metformin monotherapy. (Source: British Journal of Clinical Pharmacology)

Thrombotic microangiopathy associated with Gemcitabine use: presentation and outcome in a national French retrospective cohort
ConclusionThis large cohort confirms the severity of G ‐TMA, associated with severe renal failure and death. Edema and hypertension could be monitored in patients treated with Gemcitabine to detect early TMA. The benefit of PE or Eculizumab deserves further investigation. Development of a physiologically based pharmacokinetic model for mefloquine and its application alongside a clinical effectiveness model to select an optimal dose for prevention of malaria in young Caucasian children
ConclusionsThe PBPK and clinical effectiveness models are mutually supportive and suggest a prophylactic dose of 62.5  mg weekly in the Caucasian 5–10 kg infant population travelling to endemic countries. This dual approach offers a novel route to dose selection in a vulnerable population, where clinical trials would be difficult to conduct. (Source: British Journal of Clinical Pharmacology)
