Gentamicin in Newborn Sepsis: Once Daily Dose versus Twice Daily Dose
Gentamicin is commonly used as first line drug in neonatal sepsis. It is usually used in combination with other drugs. It is a nephrotoxic and an ototoxic drug. Long term use causes renal failure and hearing impairment. Its efficacy depends on the peak concentration, and its toxicity is related to the trough concentration. There is no set protocol of using gentamicin in neonate in Bangladesh, whether it should be used once daily or twice, it is till debatable in this perspective. In daily practice, it is not followed up trough and peak concentration of gentamicin, so it is not known the renal and hearing status of the baby...

Birth Prevalence of Congenital Cytomegalovirus (CMV) infection in a cohort of pregnant women in Bangladesh
Cytomegalovirus (CMV) is a frequent cause of congenital infection in humans in all regions of the world. In contrast to most congenital viral infections, congenital CMV infection and disease have been consistently demonstrated in populations with a high seroprevalence. Three hundred pregnant women were studied prospectively in their 1st, 2nd and 3rd trimester to determine the seroprevalence and seroconversion of CMV in pregnancy. After birth, babies were also tested for anti CMV IgM to determine the rate of birth prevalence. Anti CMV IgG and IgM tests were performed by chemiluminescence methods. All 300 (100%) pregnant wom...

Clinical and Radiographic Efficacy of Portland Cement as Pulpotomy Material in Human Primary Molar
Mineral trioxide aggregates (MTA) has been considered one of the most effective pulpotomy materials as it induces the formation of dentine at a greater rate with superior structural integrity, minimal inflammation and nominal hyperplasia. Portland cement (PC), posing the principal chemical elements with similar mechanisms of action, physical properties and biocompatibility like MTA, may be considered as effective alternative for the same. So, the objective of the study was to compare the clinical and radiographic outcome of Portland cement and mineral trioxide aggregates as pulpotomy material in primary teeth. A total of 4...

Prevalence of anti-HBc total positivity in an impoverished Urban Community in Banglades
The infection with the Hepatitis B virus (HBV) is a global health problem. Hepatitis B virus (HBV) infections are rapidly spreading in developing countries due to the lack of health education, poverty, illiteracy and Hepatitis B vaccination. No widespread population based data of HBV is available in the country ' s aspect. So, a population-based serological survey was done to determine the prevalence of the Hepatitis B core antibody total (IgM+IgG) in an impoverished Urban Community in Dhaka, Bangladesh. A descriptive cross-sectional study was conducted among 384 healthy individuals and age between 18-60 years from the urb...

Splenic Volume: Correlation between Ultrasonogram and Standard CT Measurements
Evaluation of splenic enlargement is important in the diagnosis and management of various diseases including liver disease, lymphoma and other primary or metastatic neoplasm. This cross sectional study was conducted in the department of radiology and imaging in Bangladesh Institute of Research and Rehabilitation in Diabetes, Endocrine and Metabolic Disorders (BIRDEM) from July 2007 to June 2009. The main objective of the study was to find out a more accurate sonographic measurement of splenic volume that well correlate with gold standard computed tomography (CT) volume. A total of 228 subjects were included in the study. A...

Propranolol Versus corticosteroids for Infantile Hemangiomas: A Randomized Control Trial in a Tertiary care Hospital
Infantile hemangiomas are the most common vascular tumor of infancy and childhood. Sometimes these lesions interfere with normal function& produce serious disfigurement that is unlikely to resolve on its own and then treatment is required. While evidence most supports the use of corticosteroids, there is no well- studied or Federal Drug Administration (FDA) approved systemic therapy for haemangiomas of Infancy. Dramatic improvement of complicated haemangioma by propranolol has recently been reported, but to date, details for initiating therapy, monitoring and potential risks in relation to Corticosteroids were not comp...

Helicobacter pylori cagA gene Polymorphism in Patients with Gastroduodenal Diseases
Helicobacter pylori is a genetically diverse bacterial pathogen and itsCagA gene is a major virulence factor that plays an important role in gastroduodenal pathologies. The biological function of cagA depends on tyrosine phosphorylation within the EPIYA (Glutamate-Proline-Isoleucine-Tyrosine-Alanine) motifs at the C-terminal region of the protein. This region may undergo polymorphism to give different types of EPIYA motifs. EPIYA motif diversity may provide a useful tool for prediction ofH. pylori pathogenic activity and accurate determination of number and type of cagA EPIYA motifs could identify the virulentH. pylori. Th...

Incretin Hormones and Insulin Responses During OGTT in Newly Diagnosed T2DM Patients
Several research groups have reported variable results about incretin effects of glucose-dependent insulinotropic polypeptide (GIP) and glucagon-like peptide-1(GLP-1) particularly, as well as cytokines balance in type 2 diabetes mellitus (T2DM). The present case-control prospective interventional study was conducted in 2014-2015 at Medical College for Women& Hospital, Dhaka, investigating responses of incretin hormones and insulin to oral glucose tolerance test (OGTT) in Bangladeshi subjects. Blood samples were collected from 36 OGTT positive adult T2DM patients (Pt) and 30 normal adults (NC) at ' 0 ' minute (fasting) ...

Determination of HER-2/neu gene Status by Chromogenic in Situ Hybridisation Assay on Borderline (2+) Immunohistochemistry Cases in Patients with Invasive Breast Carcinoma: An Experimental Study on Preserved Tissue
HER-2 (also known as HER-2/neu) is a member of the epidermal growth factor (EGF) receptor family. The amplification of oncogene HER-2 is presented in 20 to 30% of breast cancers and results in an increase of the protein expression. HER-2 over expression has also been shown to correlate with poor prognosis. It is associated with poorly differentiated high grade tumors with lymph node involvement, greater risk of recurrence and relative resistance to some types of chemotherapy. The receptor is however a target for treatment with anti HER-2 antibody trastuzumab (Herceptin). Immunohistochemistry analysis for HER 2 scoring is s...

Fanconi Anaemia – A Rare Case Report
Fanconi anaemia is a rare and most common form of inherited aplastic anaemia. It is mostly autosmal (except one x link) recessive disorder characterized by diverse congenital malformations, progressive pancytopenia, and predisposition to both haematological malignancy and solid tumours. Congenital malformation varies from patient to patient and may affect the skeletal system as well as organ systems. Highly variable phenotypic   presentation with clinical menifestations makes difficult for diagnosis in some cases. Chromosomal breakage study induce by Mitomycin-C (MMC)/Diepoxybutane(DEB) provide a unique cellular marker fo...
