Critical appraisal of the impact of surgical repair of type II–IV paraoesophageal hernia (POH) on pulmonary improvement: A systematic review and meta-analysis
ConclusionThis review demonstrates that POH repair is associated with improvement in pulmonary symptoms with acceptable low laparoscopic conversion rates, morbidity, mortality and recurrence rates. (Source: The Surgeon)

Adrenaline Auto-injector injuries to digits; a systematic review and recommendations for emergency management
ConclusionPhentolamine is the most effective method of reversing symptoms and treating ischaemic digits when compared to alternative therapies. Symptoms resolved much quicker when phentolamine was infiltrated into the site of injury compared to being infiltrated as a digital block. We propose a treatment algorithm for management of these injuries. Hand surgeons should be aware of AAI injuries and be able to advise on their management. (Source: The Surgeon)

Hyponatremia as a marker of complicated appendicitis: A systematic review
ConclusionsThe admission serum sodium level measurement, a routinely performed, low-cost test, should be taken into account in patients with clinical presentation compatible with acute appendicitis and suspicion of underlying complications. Future well-designed prospective diagnostic accuracy studies are required to further establish the association between hyponatremia and perforated appendicitis. (Source: The Surgeon)

Pedicle screw instrumentation with or without pelvic fixation in neuromuscular scoliosis: Outcome and complications in a series of 37 patients with a minimum 2-year follow-up
ConclusionPedicle screw fixation only for spine fusion in patients with NMS can be applied safely with reasonable complication rates. An excellent correction in all planes, a significant improvement of the pelvic obliquity and almost no loss of correction at 2 years FU were observed. (Source: The Surgeon)

Chest wall resection and reconstruction for recurrent breast cancer – A multidisciplinary approach
ConclusionA multidisciplinary oncoplastic approach for recurrent breast cancer, which includes chest wall resection and reconstruction is a useful adjunct in selected group of patients. This improves local disease control, symptoms and possibly disease-free survival. (Source: The Surgeon)

How to convert a square knot or surgeon's knot into a sliding knot and an assessment of their sliding and re-locking properties?
ConclusionThe rapid knot unlocking (RKU) technique can unlock any one of the double-stranded types of traceless knot. The suture loop tension can be adjusted at will. Bi-directional movement of the fast knot and the re-locking of sliding knot are solved. (Source: The Surgeon)

A modified Delphi process to establish future research priorities in malignant oesophagogastric surgery
ConclusionA modified Delphi process produced a list of 12 high priority research questions in OG cancer surgery. Future studies and awards from funding bodies should reflect this consensus list of prioritised questions in the interest of improving patient care and encouraging collaborative research across multiple centres. Re-do cytoreductive surgery for peritoneal surface malignancy: Is it worthwhile?
ConclusionIn well selected patients, iterative CRS and HIPEC is safe, and can provide favorable survival with low morbidity. Completeness of cytoreduction and PCI are factors that influence overall survival. (Source: The Surgeon)

A healthy decline in age- and sex- specific incidence of hip fractures in the elderly in the Republic of Ireland
ConclusionThe annual hip fracture incidence has decreased across both sexes in this study period. This may be associated with preventative measures and introduction of fracture liaison services. This study highlights the importance of investment in preventative strategies for a continued reduction in hip fracture incidence and will help to plan future services. (Source: The Surgeon)

Closed incision negative pressure therapy: Results of recent trials and recommendations for clinical practice
ConclusionciNPWT in open abdominal surgery has demonstrated promising results for reducing SSI rate in some trials however, patient selection remains unclear. Recent randomized controlled trials have failed to demonstrate benefit overall with use of ciNPWT in open abdominal surgery. Further multicentre prospective trials are needed for cost-benefit analysis and appropriate patient-selection. (Source: The Surgeon)

Ileo-anal pouch excision and permanent ileostomy – Indications and outcomes from a tertiary centre
ConclusionPouch excision is a complex, high-risk procedure that should be carried out in specialist centres. Our series shows that in such settings, good outcomes can be achieved for these patients. (Source: The Surgeon)
