Integrating Population, Health, and Environment Programs with Contraceptive Distribution in Rural Ethiopia: A Qualitative Case Study
This article examines the community‐based distribution (CBD) of family planning commodities in rural Ethiopia through EWNRA's large, multisectoral PHE program. Participants indicated that the integrated program encouraged acceptance of family planning and reduced geographic barriers to access. Through peer education and collaboration across government ministries, EWNRA leveraged integrated population–environment messages to garner support for its network of CBD providers. These integration strategies are a model for PHE programs worldwide, especially amid the global response to climate change. Because of the complex na...

The Constraints of Distance and Poverty on Institutional Deliveries in Pakistan: Evidence from Georeference‐Linked Data
While institutional deliveries in Pakistan have risen substantially over the last few years, the change has mainly occurred among the wealthy and those with access to services in urban areas. We assess the influence of economic and geographic access to health facilities on institutional deliveries by linking household survey data and georeferenced distance to facilities equipped to provide services for obstetric care in nine districts in Pakistan. Multilevel mixed‐effect logistic regression analyses show that the net effect of an increase in distance to a facility by 1 kilometer is to decrease the odds of an institutiona...

Peer Influences on Sexual Activity among Adolescents in Ghana
Little is known about the influences of peers on the sexual activity of adolescents in sub‐Saharan Africa. Better understanding of these issues could lead to more effective sexual and reproductive health interventions. Using two waves of survey data from 1,275 adolescents in two southeastern Ghanaian towns, we examine age, sex, and community differences in peer group characteristics. We also examine prospective associations between peer group characteristics and self‐reported sexual initiation and multiple partnerships during a 20‐month follow‐up period. Sex differences in peer‐context variables were small. Affil...

China's Family Planning Policies: Recent Reforms and Future Prospects
In November 2013, China announced reforms to its family planning policies whereby couples would be allowed to have a second child if either parent is an only child. The announcement garnered worldwide media coverage, and stimulated academic and popular discussion. We explore the context of the 2013 reforms, including the economic, demographic, and political motivations behind them, and speculate on their likely effect. Noting that the impact of the reforms on China's long‐term demographic future is likely to be relatively slight, we consider why more radical reform may have been difficult to implement. We offer observati...

Induced Abortions and Unintended Pregnancies in Pakistan
During the past decade, unmet need for family planning has remained high in Pakistan and gains in contraceptive prevalence have been small. Drawing upon data from a 2012 national study on postabortion‐care complications and a methodology developed by the Guttmacher Institute for estimating abortion incidence, we estimate that there were 2.2 million abortions in Pakistan in 2012, an annual abortion rate of 50 per 1,000 women. A previous study estimated an abortion rate of 27 per 1,000 women in 2002. After taking into consideration the earlier study's underestimation of abortion incidence, we conclude that the abortion rat...

Accuracy of Standard Measures of Family Planning Service Quality: Findings from the Simulated Client Method
This study, conducted within 19 public and private facilities in Kisumu, Kenya, used the simulated client method to test the validity of three standard data‐collection instruments used in large‐scale facility surveys: provider interviews, client interviews, and observation of client–provider interactions. Results found low specificity and low positive predictive values in each of the three instruments for a number of quality indicators, suggesting that the quality of care provided may be overestimated by traditional methods of measurement. Revised approaches to measuring family planning service quality may be needed ...

Reducing Unintended Pregnancies: A Microsimulation of Contraceptive Switching, Discontinuation, and Failure Patterns in France
Although the rate of contraceptive use in France is high, more than one‐third of pregnancies are unintended. We built a dynamic microsimulation model that applies data from the French COCON study on method switching, discontinuation, and failure rates to a hypothetical population of 20,000 women, followed for five years. We use the model to estimate the adjustment factor needed to make the survey data fit the demographic profile of France by adjusting for underreporting of contraceptive nonuse and abortion. We then test three behavior‐change scenarios that could reduce unintended pregnancies: decreasing method failure,...

HIV Status, Gender, and Marriage Dynamics among Adults in Rural Malawi
Awareness of and responses to HIV health risks stemming from relations between sexual partners have been well documented in sub‐Saharan Africa, but few studies have estimated the effects of observed HIV status on marriage decisions and outcomes. We study marriage dissolution and remarriage in rural Malawi using longitudinal data with repeated HIV and marital status measurements. Results indicate that HIV‐positive individuals face greater risks of union dissolution (via both widowhood and divorce) and lower remarriage rates. Modeling studies suggest that the exclusion of HIV‐positive individuals from the marriage or p...
