Huge right ventricular mass revealing a testicular nonseminomatous germ cell tumor
ConclusionsHere, we describe the world's first reported case of intraventricular metastasis from a nonseminomatous germ cell tumor of the testis which occupies almost all the right ventricular volume and extending to the root of pulmonary artery that underwent surgical resection of the ventricular mass before chemotherapy. Hyper-dominant left anterior descending coronary artery with continuation as a posterior descending artery—An extended empire
Publication date: July 2018Source: Journal of the Saudi Heart Association, Volume 30, Issue 3Author(s): Pankaj Jariwala, Edla Arjun Padma KumarAbstractHyper-dominant left anterior descending artery (LAD) is a rare coronary anomaly where LAD continues as a posterior descending artery. It is a rare coronary anomaly and there are only 19 cases reported so far in 17 case reports in the literature. Its involvement during acute coronary syndrome can be fatal as it leads to ischemia/infarction of a larger area of left and/or right ventricular myocardium. Its early recognition and management is essential with a high index of clini...

Aneurysm formation of pericardial patch in Manouguian procedure
Publication date: Available online 3 July 2018Source: Journal of the Saudi Heart AssociationAuthor(s): Hamidreza Pouraliakbar, Maryam Pourmojib, Yousef Rezaei, Saeid HosseiniAbstractPericardial patch has been used to repair cardiac defects; however, its strength as an aortoplasty patch to tolerate systemic pressure is a matter of debate. Herein, we report an aneurysmal dilatation of pericardial patch in aortoplasty. Our patient was a 56-year-old female known case of rheumatic heart disease that underwent redoes mitral and aortic valve replacements along with Manouguian aortoplasty at the age of 44 years old. After 2 months...

Transient global amnesia: Minor inconvenience or early warning sign?
Publication date: October 2018Source: Journal of the Saudi Heart Association, Volume 30, Issue 4Author(s): Erfanul Saker, James Nguyen, Neil VyasAbstractTransient global amnesia (TGA) is a clinical diagnosis and is often a diagnosis of exclusion. However, despite the benign nature of this condition, it has been associated with underlying life-threatening medical conditions (e.g., myocardial infarction, dissecting aortic aneurysm, arrhythmias). Our case report highlights the importance of early recognition of those with cardiovascular risk factors who present with acute onset altered mental status to look for underlying med...

Impact of serial echocardiography in the management of primary cardiac lymphoma
Publication date: April 2018Source: Journal of the Saudi Heart Association, Volume 30, Issue 2Author(s): Alberto Francesco Cereda, Antonella Maurizia Moreo, Paola Sormani, Benedetta De Chiara, Francesca Casadei, Michelle Zancanella, Chiara Rusconi, Roberto Cairoli, Cristina GiannattasioAbstractPrimary cardiac lymphoma (PCL) represents an extremely rare subset of extranodal lymphomas for which the primary lesion arises from the heart. The prognosis is poor, since diagnosis is often delayed and median survival is less than 12 months. We aimed at elucidating the usefulness of echocardiographic monitoring in the clinical manag...

Nonsymptomatic myocardial bridge causing systolic total narrowing of circumflex artery
Publication date: April 2018Source: Journal of the Saudi Heart Association, Volume 30, Issue 2Author(s): Fatih Aksoy, Hasan Aydın Baş, Ahmet AltınbaşAbstractMyocardial bridge is defined as the narrowing of any coronary artery segment in systole but a normal diameter in diastole. It is most frequently seen on left anterior descending (LAD) artery. Left circumflex artery (LCx) is very rare. A 62 year-old male patient presented with severe, squeezing chest pain. The electrocardiogram showed T wave inversion in V1–V4 and ST depression in DII, DIII, aVF. Coronary angiography showed complicated lesion on after S2 branches ...

Mechanolytic management of transcatheter aortic valve thrombosis
We report a challenging case of a TAVI thrombosis occurring 6 months after the procedure. Initial anticoagulation using low-molecular-weight heparin followed by thrombolytic therapy failed to both relieve symptoms and alleviate thrombosis. However, the condition of the patient deteriorated rapidly, necessitating the use of balloon valvuloplasty followed by low-dose thrombolysis. The uniqueness of the case can be summarized as follows: (1) first report of balloon valvuloplasty to manage a case of TAVI thrombosis; (2) thrombolytic therapy after balloon valvuloplasty was successful to further reduce gradient from valve t...

A huge Morgagni hernia with compression of the right ventricle
Publication date: April 2018Source: Journal of the Saudi Heart Association, Volume 30, Issue 2Author(s): Paolo Ciancarella, Fabio Fazzari, Valentina Montano, Marco Guglielmo, Gianluca PontoneAbstractA 21 year old male with no relevant medical history presented to our Institution for further assessments of a right paracardiac mass founded on a chest X-ray. Chest computed tomography revealed a wide median defect of the diaphragm at the level of xiphoid process of the sternum, with the herniation of omental fat tissue in the mediastinum. Cardiac magnetic resonance confirmed the presence of a huge hernia originating from the f...

Alhebaishi, Omar Hamoui, Taher Hassan, Adel Khalifa Hamad, Mohamed Magdy, Hani Sabbour, Sameh ShaheenAbstractClinical guidelines for the prevention of stroke in patients with nonvalvular atrial fibrillation (NVAF) are available from several international cardiology associations. Patients with NVAF in the Middle East and North Africa (MENA) region present unique challenges and opportunities related to differences in geography, practice patterns, and patient demographics that are as yet unaddresse...
