Sickle Cell Disease, Severe Acidosis, and Inhalation Injury: Case Report of Neurologically Intact Survival After Cardiac Arrest From Smoke Inhalation
We present the unusual case of complete neurologic survival after prolonged hypoxia, severe acidosis, out of hospital cardiac arrest, and exposure to high levels of carbon monoxide poisoning in a patient with sickle cell disease. The hypothesis that there might be a potential protective effect from the type of hemoglobin seen in patients with sickle cell disease in carbon monoxide poisoning is discussed. Propylthiouracil-Associated Leukocytoclastic Necrotizing Cutaneous Vasculitis: A Case Report and Review of the Literature
The purpose of this case report and review of the literature is to provide an exploration of the clinical symptoms, diagnosis, prevention, and management of propylthiouracil (PTU)-associated vasculitis in the intensive care setting. A PubMed search of the available literature was conducted using the MeSH search terms “propylthiouracil” and “vasculitis.” The literature search returned 121 articles. Twenty-five were excluded because they were not in English. Fifty-nine case reports or case studies describing PTU-associated vasculitis were included. Data extracted from each case study included patient age, sex, autoim...

Innovative Techniques for Maximizing Limb Salvage and Function
Concurrent injuries to multiple extremities present unique challenges to the reconstructive surgeon. The primary goal in such scenarios is to optimize functional outcomes. The goal of this article is to present an overview of various techniques necessary to provide sufficient soft tissue and preserve amputation limb lengths and function. The concept of innovative techniques for maximizing limb savage and function is presented using an index patient with multiple extremity third- and fourth-degree burn injuries resulting in nonsalvageable lower extremities and severe left-hand wounds. A review of other potential innovative ...

Surface Disinfectants for Burn Units Evaluated by a New Double Method, Using Microorganisms Recently Isolated From Patients, on a Surface Germ-Carrier Model
Assessment methods of surface disinfection based on international standards (Environmental Protection Agency, European Norms, etc) do not correspond to hospital reality. New evaluation methods of surfaces disinfection are proposed to choose the most suitable disinfectant to act against clinically relevant microorganisms detected on the surfaces of burn units. 1) “Immediate effect”: 6 products were compared using a glass germ-carrier and 20 recently isolated microorganisms from different patients in the intensive care units. Disinfectants were applied with microfiber cloths. Log10 reductions were calculated for colony f...

Theory-Based Cartographic Risk Model Development and Application for Home Fire Safety
There is a gap in the use of predictive risk models to identify areas at risk for home fires and burn injury. The purpose of this study was to describe the creation, validation, and application of such a model using a sample from an intervention study with parents of newborns in Jefferson County, KY, as an example. Performed was a literature search to identify risk factors for home fires and burn injury in the target population. Obtained from the American Community Survey at the census tract level and synthesized to create a predictive cartographic risk model was risk factor data. Model validation was performed through cor...

Functional Exercise Capacity in Children With Electrical Burns
Electrical burns are a severe form of thermal injury extending deep into tissue. Here, we investigated the effect of electrical burns on metabolic rate, body composition, and aerobic capacity. We prospectively studied a cohort of 24 severely burned children. Twelve patients had a combination of electrical and flame burns and 12 matched controls had only flame burns. Endpoints were cardiopulmonary fitness (maximal oxygen consumption [VO2]), muscle strength (peak torque per body weight), body mass index, lean body mass index, and days of myoglobinemia (≥500 mg/dl). Demographics of both the groups were comparable. The ele...

Surgical Management of Levamisole-Adulterated Cocaine Induced Soft Tissue Necrosis: Case Study and Treatment Algorithm
Levamisole is an increasingly common cocaine adulterant that can cause severe and rapid onset cutaneous vasculitis in humans. While most cases may be managed conservatively, we describe a series of patients in whom the extent of skin and soft tissue necrosis mandated surgical intervention. A retrospective review of all patients admitted to one of two regional burn centers between 2006 and 2016 for soft tissue necrosis after exposure to levamisole-adulterated cocaine was included in our study. Ten patients, majority female (9/10) with an average age of 43.4 years (range 31–57), were included. Cocaine usage before presenta...

Rehabilitation Research Priorities: The Next 10 Years
A core set of foundational themes are needed to develop the next decade of rehabilitation research priorities to optimize burn recovery. These themes include integrating burn survivors into rehabilitation research, measuring the right things, utilizing technology, learning from other fields of medicine, translating bench research to rehabilitation, harnessing big data, capitalizing on past successes, embracing social media, and expanding partnerships. It is the challenge of the burn care community to utilize these foundational themes to champion the field of burn rehabilitation research. (Source: Journal of Burn Care and Research)

Burn Care of the Elderly
As part of the State of Science Meeting, care in elderly was one of the foci. This “white paper” deriving from this meeting indicates advances in the field of burn injuries in the elderly but more importantly areas that are in need of novel insights and investigations. The group created specific segments of a patient’s course pre- and postinjury to selectively identify specifics we need to improve on. Tasks were created that were assigned to volunteers of the group and are given in detail in this article. The overarching aim is to initiate substantial momentum among the members of the American Burn Association to imp...

Burns in Children
Children have a great deal to gain from recent and future advances in burn care. A very broad range of realized and potential developments are involved. These will be very briefly reviewed within the context of four areas: 1) early evaluation and care issues, 2) acute surgical and critical care issues, 3) rehabilitation and reconstruction issues, and 4) organizational and outcomes issues.
