Predictors and Outcomes of Crossover to Surgery from Physical Therapy for Meniscal Tear and Osteoarthritis: A Randomized Trial Comparing Physical Therapy and Surgery
Conclusions: Subjects who crossed over to APM had presented with a shorter symptom duration and greater baseline pain than those who did not cross over from PT. Subjects who crossed over had rates of surgical success similar to those of the patients who had been randomized to surgery. Our findings also suggest that an initial course of rigorous PT prior to APM may not compromise surgical outcome. Level of Evidence: Prognostic Level II. Effects of Teriparatide Compared with Risedronate on Recovery After Pertrochanteric Hip Fracture: Results of a Randomized, Active-Controlled, Double-Blind Clinical Trial at 26 Weeks
Conclusions: Teriparatide was associated with less pain and a shorter time to complete the TUG test between 6 and 26 weeks compared with risedronate. Other fracture-recovery outcomes were similar. The results should be interpreted with caution as these were secondary end points. Level of Evidence: Therapeutic Level II. Overlapping Surgery in the Ambulatory Orthopaedic Setting
Conclusions: Overlapping surgery yields equivalent patient operating room time, procedure time, and 30-day complication rates as non-overlapping surgery in the ambulatory orthopaedic setting. Further investigation is warranted for inpatient orthopaedic procedures and across all orthopaedic subspecialties. Level of Evidence: Therapeutic Level III. See Instructions for Authors for a complete description of levels of evidence. (Source: JBJS)

The 2015 AOA North American Traveling Fellowship
The North American Traveling Fellowship (NATF) is one of the flagship tours of the American Orthopaedic Association (AOA). NATF is geared toward advancing the careers of young orthopaedic surgeons through the promotion of clinical, scientific, and social exchange. The 2015 tour had a major emphasis on the development of leadership in the field of orthopaedics through close interactions and meetings with departmental chairs and administrators, as well as with hospital and medical school leaders. The 2015 NATF tour was in the Midwest corridor of Canada and the United States. The 5 fellows included specialists in spine, traum...

Levels of Influence: Habituation and the Prevalence of Declared Conflicts of Interest
Conclusions: Industry relationships were common in studies in NEJM and JBJS, but rare in Pediatrics. The high prevalence of conflicts of interest may weaken the meaning of declarations and undermine any bias-reducing effect intended by the disclosure system. (Source: JBJS)

Understanding and Addressing the Global Need for Orthopaedic Trauma Care
The burden of musculoskeletal trauma is high worldwide, disproportionately affecting the poor, who have the least access to quality orthopaedic trauma care. Orthopaedic trauma care is essential, and must be a priority in the horizontal development of global health systems. The education of surgeons, nonphysician clinicians, and ancillary staff in low and middle income countries is central to improving access to and quality of care. Volunteer surgical missions from rich countries can sustainably expand and strengthen orthopaedic trauma care only when they serve a local need and build local capacity. Innovative busin...

Trends in the Incidence of Clavicle Fractures and Surgical Repair in Sweden: 2001-2012
Conclusions: Although the incidence of clavicle fractures increased in Sweden between 2001 and 2012, the rate of surgical treatment of clavicle fractures increased much more than could be expected solely based on the increase in the fracture incidence. The observed changes in the rates of surgery require additional studies since there is still controversy regarding the indications for surgical treatment. (Source: JBJS)

Tibiofemoral Contact Mechanics with Horizontal Cleavage Tear and Resection of the Medial Meniscus in the Human Knee
Conclusions: Resection of meniscal tissue forming the inferior leaf of an HCT resulted in substantially decreased contact area and increased contact pressure. Additional resection of the superior leaf resulted in a further significant decrease in contact area and increase in contact pressure in the medial compartment. Clinical Relevance: Repair or minimal resection of meniscal tissue of an HCT may be preferred to complete leaf resection to maintain knee tibiofemoral contact mechanics. (Source: JBJS)

Long-Term Outcome of Step-Cut Ulnar Shortening Osteotomy for Ulnar Impaction Syndrome
Conclusions: The step-cut ulnar shortening osteotomy provides ample bone-to-bone contact and simplifies control of rotation. Stable internal fixation with standard techniques allowed an early return to functional activities. Palmar placement of the plate diminishes the need for plate removal. This is a simple and less expensive technique for ulnar shortening that does not require the use of special instrumentation in patients with ulnar impaction syndrome. Level of Evidence: Therapeutic Level IV. See Instructions for Authors for a complete description of levels of evidence. (Source: JBJS)
