Intrapericardial left-sided ectopic thyroid mass supplied by the left circumflex artery
We report a 47-year old female patient with an intrapericardial mass mounting the left atrial wall. Two prominent vessels, both originating from the left circumflex artery (LCX), supplied the tumour. Owing to the compression of the left atrium and the left upper pulmonary vein, the patient was highly symptomatic with exertional dyspnoea and ventricular extrasystoles. Histopathological examination showed regressive thyroid adenoma with no signs of malignancy. Complete resection was feasible. Non-anastomotic rupture of the Valsalva graft 6 years after undergoing the Bentall procedure

Selective, segmental decalcification: a safe alternative to extensive debridement of a severely calcified annulus during repair of mitral regurgitation

Split-graft technique in neonatal heart transplant for aortic atresia
We describe a neonate with aortic atresia and hypoplastic aorta, listed for heart transplant after extracorporeal membrane oxygenation resuscitation and ductal stenting. The donor aorta was detached from the graft, after an isolated arch reconstruction prior to the transplant itself in a routine fashion. To the best of our knowledge, this is the first reported case of neonatal arch reconstruction before transplantation performed with grafts from the same donor in a split-way strategy. An unusual fenestration in single-stage Fontan operation
We present one such child with situs solitus, dextrocardia, DORV, hypoplastic right ventricle, large ventricular septal defect, severe pulmonic stenosis, extremely small right atrium and left juxtaposed atrial appendages, who underwent Fontan operation. We created an unusual fenestration between left pulmonary artery and juxtaposed right atrial appendage on the left side, due to anatomic complexity. Short-term results are encouraging. The experimental validation of free fat grafts in thoracic surgery
We evaluated the viability of free fat grafts in the thoracic cavity using 3-month old male swine (n = 2). After left caudal lobectomy, 1–3 g of subcutaneous fat tissue harvested via the thoracotomy site was implanted in the chest cavity. At re-thoracotomy 6 weeks after implantation, all of the implanted fat grafts (n = 15) were found to have closely adhered to the parietal pleura and visceral pleura. There was a significant decrease by ~30% in the weight of the fat grafts after implantation. Improved cognitive flexibility after aortic valve replacement surgery
CONCLUSIONS There was a marked improvement in cognitive performance following AVR surgery, specifically in executive control functions indicating improved cognitive flexibility. Cognitive improvement, as opposed to the typical trajectory of cognitive decline in an elderly population with vascular disease, emphasizes the importance of these results and points to AVR surgery as having potential benefits on brain health in general. Partial replacement of left hemidiaphragm in dogs by either cryopreserved or decellularized heterograft patch
CONCLUSIONS The gross healing process in the decellularized heterograft is similar to the cryopreserved diaphragm but with fewer inflammatory cells and foreign body granulomas on histology. Both of them can be used instead of bioprostheses with regard to the fact that the decellularized patch technique is more complex and expensive. Pressure level required during prolonged cerebral perfusion time has no impact on neurological outcome: a propensity score analysis of 800 patients undergoing selective antegrade cerebral perfusion
CONCLUSIONS We hereby present SACP data as a real-time curve, based on a large patient cohort containing a total of 800 patients. Our perfusion strategy employing a lower selective cerebral perfusion pressure (~50 vs ~80 mmHg) is not associated with a higher rate of neurological complications during aortic surgery. Early and mid-term results after hybrid total arch repair of DeBakey type I dissection without deep hypothermic circulatory arrest
CONCLUSIONS For patients with DeBakey type I dissection, the hybrid total arch procedure can be safely adopted with good mid-term results and with low morbidity and mortality.
