Clinical outcomes and direct costs after transcatheter aortic valve implantation in French centres: a longitudinal study of 1332 patients using a national database
CONCLUSIONS Mortality at 3 years is consistent with other published studies. After TAVI, hospitalization stays in both acute and rehabilitation settings, and the associated costs do not appear to be reduced compared with the year preceding TAVI. The total cost for patients undergoing TAVI is substantial at 3 years. (Source: Interactive CardioVascular and Thoracic Surgery)

Time course of left atrial reverse remodelling after mitral valve surgery and the impact of left ventricular global longitudinal strain in patients with chronic severe mitral regurgitation
CONCLUSIONS The majority of LARR after MV surgery occurred during the early postoperative period. LV-GLS, age and LA volume at surgery determined the degree of early LARR after MV surgery in patients with chronic severe MR. (Source: Interactive CardioVascular and Thoracic Surgery)

Predicting renal replacement therapy after cardiac valve surgery: external validation and comparison of two clinical scores
CONCLUSIONS Both models provided a certain clinical significance, allowing one to discrimination between patients with higher or lower risks of the postoperative need of renal replacement therapy within the present study population. However, it was not suitable for estimating the real incidence of the need for postoperative renal replacement therapy with sufficient precision in the study sample. Direct clinical use in our centre should be considered cautiously. (Source: Interactive CardioVascular and Thoracic Surgery)

Incidence and predictors of new-onset atrioventricular block requiring pacemaker implantation after sutureless aortic valve replacement
CONCLUSIONS NO-AVB is a frequent complication following SU-AVR, and its incidence strongly depends on the surgical technique. Baseline first-degree AVB and LaQD independently predict NO-AVB and should be considered when deciding the duration of postoperative electrocardiographic monitoring. A modified Cox maze IV procedure: a simpler technique for the surgical treatment of atrial fibrillation
CONCLUSIONS The MCMP-IV is an effective surgical procedure for the treatment of AF. In certain patients, such as those with anatomic variations of the pulmonary veins, the MCMP-IV is simpler than the CMP-IV. CLINICAL TRIALS REGISTRATION ID ChiCTR-TRC-12002742. Endoscopic atrioventricular valve surgery in adults with difficult-to-access uncorrected congenital chest wall deformities
CONCLUSIONS EPAAVVS in adults with uncorrected CCWD is safe, feasible and durable and can successfully be performed by experienced teams to achieve Haller index and correction index scores of up to 3.3 and 38.3%, respectively, with favourable long-term clinical and echocardiographic outcomes. The mere presence of uncorrected CCWDs should not deter surgeons from offering these patients the full benefits of minimally invasive cardiac surgery. Perioperative factors associated with short- and long-term outcomes after tricuspid valve replacement
CONCLUSIONS This study identified a number of factors that were independently associated with short-term mortality, short-term LOS and long-term mortality of patients after TVR. (Source: Interactive CardioVascular and Thoracic Surgery)

Native lung pneumonectomy for post-transplantation lymphoproliferative disorder refractory to rituximab following contralateral lung transplantation
We report a PTLD case of high-grade, B-cell lymphoma following contralateral single-lung transplantation. The disease involved the liver, right kidney and right native lung. While the PTLD affecting the abdominal organs regressed with rituximab chemotherapy, the native lung disease progressed and was treated surgically (right pneumonectomy). Some aspects are unique in this case: (i) different response to medical treatment between lung and abdominal organs; (ii) absolute absence of involvement of the native lung and (iii) surgical treatment with a pneumonectomy, still very rarely described in the literature. We hypothesized...

Rare cause of central cyanosis: right pulmonary artery to left atrial fistula
A fistula between the right pulmonary artery and the left atrium is a very rare cause of central cyanosis. A clinical diagnosis is often made by first excluding other common causes and then performing diagnostic catheterization. Surgical closure offers a definitive cure for this rare congenital anomaly. Giant dissecting ventricular septal haematoma associated with critical congenital heart disease
We report a case of a giant interventricular septal haematoma after surgical correction of a critical pulmonary valve stenosis in a newborn with intact ventricular septum. Control echocardiography showed complete resolution of the haematoma at 6-month follow-up. (Source: Interactive CardioVascular and Thoracic Surgery)

Mediastinal paraganglioma fed by the left circumflex artery
A 60-year-old male patient who previously underwent carotid and jugular paraganglioma resections was referred because of a mediastinal recurrence at the root of the great vessels. Coronary angiography confirmed the circumflex artery of the left coronary artery as the feeding artery of the tumour. The patient underwent surgery due to the tumour's location and malignant potential. Upon mass resection, histopathological examination characterized the tumour as a secondary paraganglioma. Neuroendocrine tumours arising from chromaffin tissues at the extra-adrenal paraganglions of the autonomic nervous system are termed paragangl...

Suprasternal innominate artery cannulation for reoperative aortic surgery: a technical note
Suprasternal cannulation of the innominate artery in aortic reoperations may be useful in specific situations. Over a period of 3.5 years, 9 patients (6 males, average age = 49.2 ± 16.1 years) underwent suprasternal cannulation prior to resternotomy. Cannulation was performed using a side graft. All operations were successfully completed. Two patients died after surgery because of coagulopathy and multiorgan failure. There were no complications related to access or technique, and no site complications were detected during follow-up. Suprasternal cannulation of the innominate artery may play a role in selected reoper...

In patients with resectable non-small-cell lung cancer, is video-assisted thoracoscopic segmentectomy an appropriate alternative to video-assisted thoracoscopic lobectomy?
A best evidence topic in thoracic surgery was written according to a structured protocol. The question addressed was ‘In patients with resectable non-small-cell lung cancer, is video-assisted thoracoscopic segmentectomy an appropriate alternative to video-assisted thoracoscopic lobectomy?’ Two hundred papers were found using the reported search, of which 8 represented the best evidence to answer the clinical question. The authors, journal, date and country of publication, patient group studied, study type, relevant outcomes and results of these papers are tabulated. In one meta-analysis identified, there was no...

Does induction chemoradiotherapy increase survival in patients with Pancoast tumour?
A best evidence topic in cardiothoracic surgery was written according to a structured protocol. The question addressed was whether induction (neoadjuvant) chemoradiotherapy (iCRT) compared with other therapeutic strategies improves survival in patients with Pancoast tumours. Altogether 248 papers were identified using the below-mentioned search. Ten of them represented the best evidence to answer the clinical question. The author, journal, date and country of publication, patient group, relevant outcomes and weaknesses were tabulated. Four studies were retrospective comparative studies of induction chemoradiotherapy and su...
