Hospital centralization and performance in Denmark —Ten years on
Denmark implemented a major reform of the administrative and political structure in 2007 [1,2,3,32,41]. At that time, the country had three administrative levels – the state, county and municipal – each with the authority to levy taxes. The background for the reform was that the administrative structure was seen as being composed of too many small units at both the municipal and county levels to be able to provide services of a satisfactory quality. In p articular, it was a cause for concern that hospitals with a small volume of surgical patients were not able to provide surgery of a high quality due to limited surgeon...

Health policies for the reduction of obstetric interventions in singleton full-term births in Catalonia
In recent years, the rate of surgical procedures carried out in order to assist childbirth has substantially increased [1]. Data recorded in different reports and international databases [2,3] show an increase in the rate of caesareans with high variability between countries. (Source: Health Policy)

Strategic Purchasing in Practice: Comparing Ten European Countries
Over the past few decades, policymakers across high and middle income countries have grown interested in the role that strategic purchasing could theoretically have in optimizing the cost-effective provision of healthcare services while simultaneously maximizing population health [1]. Purchasing is the process of allocating pooled funds to health care providers, whether within a NHS system with a purchaser-provider split (i.e. England), through contracts with insurance funds as in social insurance (i.e. (Source: Health Policy)

Variations in non-prescription drug consumption and expenditure: Determinants and policy implications
Cross-national variations in health care expenditure and its growth rate are some of the key aspects investigated in the literature through international (macro studies), interregional (meso studies) and local (micro studies) comparisons. A further element profiled in the literature is patient behavior in terms of healthcare resource use (e.g., distribution of costs per age, gender, etc.) [1]. (Source: Health Policy)

Reconciling a “pleasant exchange” with evidence of information bias: A three-country study on pharmaceutical sales visits in primary care
Physicians need timely access to balanced, accurate and evidence-based drug information to inform prescribing decisions [1]. In a survey of physicians in the United States (US) in 2006 –07, most (76%) agreed that sales representatives are a valuable source of information, especially on new drugs [2]. Information provided by sales representatives is often selective, with inadequate mention of possible harm [1,3,4]. This situation may lead to suboptimal prescribing with potential negative consequences for patient health. (Source: Health Policy)

High feeding dependence prevalence in residents living in Italian nursing homes requires new policies: Findings from a regionally based cross-sectional study
Different predictors have been documented as affecting nursing home (NH) admissions among the elderly and dependence in more than three activities of daily living (ADLs) has been identified as the strongest [1,2]. Specifically, some early- (dressing and personal hygiene) and mid-loss ADLs (toileting, transferring and locomotion [3,4]) have been indicated as the strongest predictors of subsequent NH placement [1]. By contrast, some late-loss ADLs such as eating [5] usually deteriorate in later stages of life, during the in-NH stay. (Source: Health Policy)

Promoting health-enhancing physical activity in Europe: Current state of surveillance, policy development and implementation
Physical activity is at the core of health and well-being. The benefits of physical activity – including reduced risk of noncommunicable diseases and lower levels of stress, anxiety, and depression – are well known [1]. The World Health Organization (WHO) recommends that adults (including elderly) engage in at least 150 min of moderate-intensity aerobic physical activity each week [2] . Figures from European Union (EU) countries indicate that six in every 10 people above 15 years of age never or seldom exercise or play any sports, and more than half never or seldom engage in other kind of physical activity, such as c...
