Abstract 144: Nuclear Stress Testing- Exercise vs. Chemical [Session Title: Poster Session AM]
Conclusion: There has been a marked increase in CST with SPECT performed at CAMC between 2007-2016. While most patients may have the physical capacity and baseline EKG characteristics to qualify for an ETT with SPECT, a clear explanation for this pattern remains unknown. A possible analysis of changes in patient characteristics over this time may provide some insight into this trend. (Source: Circulation: Cardiovascular Quality and Outcomes)

Abstract 143: Pearls and Pitfalls of Performance-linked Reimbursement Plans for Novel Drugs: The Case of Sacubitril-Valsartan [Session Title: Poster Session AM]
Conclusion: PLR plans require outcomes tightly associated with treatment value and minimally influenced by unrelated uncertainty. Both sides must agree on a predictable population and limit asymmetric selection. For rare outcomes, plans should evaluate population event rates or surrogate measures that consistently reflect the clinical benefit. (Source: Circulation: Cardiovascular Quality and Outcomes)

Abstract 142: Engaging Patients Throughout the Health System: A Landscape Analysis of Top Research Institutions and Cold Call Policies [Session Title: Poster Session AM]
Conclusion: The approach to engaging patients in large, research intensive health systems varies significantly. As healthcare has evolved with more virtual visits, team-based care or system-based care, the engagement of potential research participants should similarly evolve while preserving patient privacy and autonomy. Potential solutions include opt-in or opt-out approaches for future contact, replacement of cold call policies with best practices, or a team based collaborative approach between physicians and researchers to guide recruitment decisions. Abstract 141: Why Did My Patient Die? Deaths Following Coronary Angioplasty [Session Title: Poster Session AM]
Conclusions: Coronary PCI may be life saving. However, procedural urgency may limit knowledge of and optimization of risks. When possible, a more holistic discussion with the pt and family, giving careful consideration to the risks and benefits of angioplasty, goals of therapy and limitations of care should take place pre procedure. It is also important to be able to identify and aggressively treat the post procedure complications commonly associated with mortality. (Source: Circulation: Cardiovascular Quality and Outcomes)

Abstract 140: Antihypertension Treatment Disutility Among U.S. Adults [Session Title: Poster Session AM]
Conclusion: In contrast to guidelines which recommend lifestyle changes such as exercise as first line, people’s preferences indicated that they would be more inclined to take a daily pill or tea to prolong their life. Substantial numbers of respondents desired gains in life years that were beyond what each treatment could maximally provide. (Source: Circulation: Cardiovascular Quality and Outcomes)

Abstract 139: Examining the Effect of Post-Hospital Syndrome on Outcomes in Patients Discharged After an Acute Coronary Syndrome [Session Title: Poster Session AM]
Conclusions: NPO status and tests at irregular hours were correlated with negative short and longer-term outcomes. Efforts should be made to limit these inpatient stressors or to stabilize patients prior to discharge. Future research on post hospital syndrome is warranted to better understand potential longer-term relationships and to better plan for care transitions. Abstract 138: Wooden Chest Syndrome Complicated by Cardiopulmonary Arrest: A Rare Case of Fentanyl Toxicity [Session Title: Poster Session AM]
Conclusion: CPA is a rare but fatal complication of WCS. WCS can occur with low doses of fentanyl even in patients who have previously tolerated higher doses. (Source: Circulation: Cardiovascular Quality and Outcomes)

Abstract 137: A Community Initiative to Reduce Blood Pressure in Latinos: The San Antonio AHA Check.Change.Control Initiative [Session Title: Poster Session AM]

Abstract 136: Blood Pressure Control According to Race and Gender in a University Cardiology Clinic [Session Title: Poster Session AM]
Conclusion: Highest documented BP was observed in African American females with treated hypertension. Lack of compliance with medications is unlikely as these patients had insurance and their LDL levels were adequately controlled with medications. Slightly higher body weight in AAF, though not statistically significant, may have contributed to a higher BP. White-coat-hypertension is another potential explanation, which has been previously reported to be prevalent in African American females. Further studies with ambulatory BP monitoring may help better explain this observation. AAF remain at very risk for cardiovascular di...

Abstract 135: Extended Thromboprophylaxis With Betrixaban is Cost-effective in Acutely Ill Medical Patients [Session Title: Poster Session AM]
Conclusions: Based on the rates of clinical events reported in the APEX study, extended-duration thromboprophylaxis with betrixaban from hospital admission through post-discharge in acutely ill medical patients is likely to prevent additional deaths and reduce costs compared with standard-duration thromboprophylaxis with enoxaparin. Abstract 134: Does Low-Level Cumulative Lead Play a Role in Resistant-Hypertension? [Session Title: Poster Session AM]
Conclusion: Lead has been long-studied for its effect on blood pressure. Yet, lead has not previously been assessed for the role it plays in clinical outcomes. Difficulty in attaining goal blood pressure may be influenced by environmental exposures. Our study demonstrates an increased association between tibia lead and resistant-HTN status, with an OR of 1.27 per one IQR increase in tibia lead. Tibia lead represents a novel risk factor for resistant-HTN. Future research should consider screening and mitigation strategies for populations with resistant-HTN exposed to long-term low-levels of lead. (Source: Circulation: Cardi...

Abstract 133: Using Administrative Data Registries and Clinical Data Registries to Identify Discrepancies and Improve Data Quality [Session Title: Poster Session AM]
Conclusion: Comparing administrative and clinical data registries data is an effective quality tool to identify discrepancies in each source of data. Sharing discrepancies with hospitals results in improved understanding of data standards and data quality. (Source: Circulation: Cardiovascular Quality and Outcomes)

Abstract 132: Endovascular First Treatment is Associated With Improved Amputation Free Survival in Patients With Critical Limb Ischemia [Session Title: Poster Session AM]
Conclusions: Patients with CLI have multiple comorbidities, and initial surgical bypass is associated with poorer quality of amputation free survival compared to an endovascular first approach. Treatment in high volume centers improves amputation free survival. This may be due to improved resources at high volume centers. (Source: Circulation: Cardiovascular Quality and Outcomes)

Abstract 131: Recurrent Acute Cardiovascular Events and Statin Use [Session Title: Poster Session AM]
Conclusion: While statin use substantially increased after an index CV event, nearly 40% of patients remained untreated, increasing their recurrent CV event risk by 76%. Initiation and continuation of statin therapy after CV events remains a large public health opportunity for patients with ASCVD. (Source: Circulation: Cardiovascular Quality and Outcomes)

Abstract 130: Outcomes of Transcatheter Aortic Valve Replacement in Patients With Chronic Kidney Disease [Session Title: Poster Session AM]
Conclusions: A significant proportion of TAVR recipients suffer from chronic kidney disease. These patients were more likely to experience vascular complications and worsening of renal dysfunction, a prolonged intensive care unit and hospital length of stay, and an increased one-year mortality. Additional prospective randomized studies of this important patient population are warranted. (Source: Circulation: Cardiovascular Quality and Outcomes)
