A hypokalaemic woman with nephrocalcinosis: rebirth of old knowledge
We describe a 38-year-old woman who presented with thirst, intermittent carpopedal spasm, paresthaesia of both hands and progressive weakness of lower extremities for two years. She had a history of chronic hypokalaemia of unknown cause with intermittent potassium supplementation for 7–8 y and bilateral nephrocalcinosis notable for one year. She denied vomiting, diarrhoea or use of laxatives, alcohol or diuretics. Her blood pressure was normal. Laboratory investigations showed hypokalaemia (2.7 mmol/L) and metabolic alkalosis (HCO3– 32.6 mmol/L, pH 7.46). Two random urine samples both showed a consistently high...

Lipoprotein X in a patient with cholestasis and hypertriglyceridaemia
We report a case of severe hypertriglycaeridemia of 149 mmol/L in a 36-year-old man with type 2 diabetes who presented to the surgical ward with abdominal pain due to pancreatitis and developed acute cholestasis, jaundice and eruptive xanthomata. His triglycerides improved to 3.8 mmol/L with sliding scale insulin within two weeks of in-hospital stay. However, his total cholesterol remained raised at 23.7 mmol/L. The lipoprotein electrophoresis confirmed the presence of lipoprotein X associated with bile obstruction, which contributed to an increase in total cholesterol. The total cholesterol normalized on improvement of hi...

The insulin-like growth factor-system in a patient with diffuse large B-cell non-Hodgkin's lymphoma and lactic acidosis
We present a case of a 79-year-old man with a diffuse large B-cell lymphoma and lactic acidosis. Initially, the patient was successfully treated according to the R-CHOP scheme. After recurrence of disease, the patient was treated according to a protocol of the Dutch-Belgian Haemato-Oncology Group (HOVON-85 study). Eleven months after completion of the last therapy, the patient still appeared to be in complete remission. Serum levels of IGFs, pro-IGF-IIE[68–88], IGF binding proteins (IGFBPs)-1 to -4, acid labile subunit (ALS), as well as ternary IGF-I-IGFBP-3-ALS complex formation, were determined in samples taken bef...

Effect of an educational intervention on requesting behaviour by a medical admission unit
Conclusions This process highlighted inappropriate requesting behaviour from the MAU which, following a process of investigation and education, resulted in a significant change with an associated cost benefit. (Source: Annals of Clinical Biochemistry)

Extensive bowel resection in infancy is another cause of increasing plasma alphafetoprotein concentration
Conclusion Rising plasma AFP concentrations post extensive bowel resection in infants is a new finding which is possibly due to intestinal regeneration. (Source: Annals of Clinical Biochemistry)

Establishment of paediatric age-related reference intervals for serum prolactin to aid in the diagnosis of neurometabolic conditions affecting dopamine metabolism
Conclusions This is the first published study to report age-related prolactin reference intervals spanning all paediatric age groups where the data are age partitioned in infants under one year of age using an ‘indirect’ approach. It is envisaged that these reference intervals will be used to aid in the selection of patients presenting with features of central dopamine deficiency for lumbar puncture. Also, these reference intervals have the potential for treatment monitoring of patients with diagnoses of inborn errors of dopamine biosynthesis. (Source: Annals of Clinical Biochemistry)

Analysis of plasma 3-methoxytyramine, normetanephrine and metanephrine by ultraperformance liquid chromatography-tandem mass spectrometry: utility for diagnosis of dopamine-producing metastatic phaeochromocytoma
Conclusions The LC-MS/MS method enables accurate, selective and sensitive measurements of catecholamine O-methylated metabolites that should be particularly useful for screening and management of dopamine-producing metastatic PPGLs. (Source: Annals of Clinical Biochemistry)

Characterization of the perfusate proteome of human donor kidneys
Conclusions: We provide the first description of the renal perfusate proteome and present preliminary evidence of differentially expressed biomarkers in human donor kidneys with different levels of acute ischaemic injury. Their diagnostic value for the selection of marginal kidneys in clinical transplantation should be determined in future studies. (Source: Annals of Clinical Biochemistry)

Demography and biochemistry of 2800 patients from a renal stones clinic
Conclusions Risk factors for stones differ between men and women, change with ageing and in some may have a genetic basis. The role of phosphaturia merits further exploration. (Source: Annals of Clinical Biochemistry)

Elevated cardiac markers are associated with higher mortality in patients after transjugular intrahepatic portosystemic shunt insertion
Conclusions Measurement of cardiac markers, mainly hs-cTnT and FABP, may be useful for mortality prediction in cirrhotic patients after TIPS. Cardiac markers are better mortality predictors than other risk factors such as age, gender or Child–Pugh score. (Source: Annals of Clinical Biochemistry)

Association between C-reactive protein, pro-oxidant-antioxidant balance and traditional cardiovascular risk factors in an Iranian population
Conclusions This study showed that among Iranian subjects, the inflammatory marker, hs-CRP, was strongly and positively associated with a marker of oxidative stress and also with several traditional risk factors of CVD. Moreover, the impact of traditional cardiovascular risk factors on hs-CRP concentrations and PAB values differed. (Source: Annals of Clinical Biochemistry)

Systematic review on the association between F2-isoprostanes and cardiovascular disease
Conclusions High levels of F2-isoprostanes in urine or blood may be a non-specific indicator of CVD. However, further population-based studies are needed. In addition, multivariable analyses are required for future studies to control confounding and improve classification accuracy. (Source: Annals of Clinical Biochemistry)
