Role of mpMRI PSA Density and PI-RADS Score in Predicting Upstaging in Men on Active Surveillance
Conclusion A combination of PI-RADS score and prostate specific antigen density predicts patients at risk of being upstaged at surveillance biopsy. Conversely, this combination may help determine which men may safely forgo biopsy. (Source: Urology Practice)

Trends in the Setting and Cost of Ambulatory Urological Surgery: An Analysis of Five States in the Healthcare Cost and Utilization Project
Conclusions Ambulatory urological surgery represents 5% of all surgical cases, but 7.6% of costs. The rate of procedures is increasing steadily with performance in ASCs outpaced by those in HOPD. The cost of ambulatory urological surgery is rising out of proportion to explanation by inflation, patient factors, or case-mix. (Source: Urology Practice)

Operating Room Supply Cost Awareness: A cross-sectional analysis
Conclusions Trainees and faculty generally have poor knowledge of operating room supply costs. In our academic setting, we noted an interest among both faculty and residents to make cost data more accessible. These data would provide an opportunity for surgeons to act as cost arbiters in the operating room. (Source: Urology Practice)

Trends in the Authorship of Peer Reviewed Publications in the Urology Literature
Conclusions There is a global trend toward more authors per article in urology publications for original research as well as review articles, and across each of the individual journals evaluated. An increase in author count has also been associated with increased citations and measures of article impact. (Source: Urology Practice)

The Representation of Women in Urological Leadership
Conclusions Overall, women are fairly represented in a broad cross-section of leadership roles. Women are less well represented in the upper echelon of urological leadership, especially department chairs. (Source: Urology Practice)

Reduction in Opioid Prescribing Using a Postoperative Pain Management Protocol following Scrotal and Subinguinal Surgery
Conclusions Evaluation of postoperative pain management revealed excessive prescribing of opioid medications compared to actual usage. Our protocol resulted in a significant decrease in opioid prescribing without compromising management of postoperative pain. Adjunct treatments for pain, including scrotal support, ice packs, elevation and nonsteroidal anti-inflammatory drugs, may improve postoperative pain control without increasing opioid usage. The combination of enhanced patient education and reduced opioid prescribing may result in decreased opioid use, opioid abuse and medication costs. (Source: Urology Practice)

Comparative Outcomes of Conventional and Miniaturized Percutaneous Nephrostolithotomy for the Treatment of Kidney Stones —Does a Miniaturized Tract Improve Quality of Care?
Conclusions Conventional and mini percutaneous nephrostolithotomies are effective methods of removing renal stones 1 to 3.5 cm in greatest dimension. There is no difference in residual stone volume, postoperative analgesic use or operative time between the 2 modalities, but blood loss is less in the mini percutaneous nephrostolithotomy group. The Landscape of Coverage for Fertility Preservation in Male  Pediatric Patients
Conclusions Coverage of fertility preservation is extremely limited in scope of benefits and the number of states that require such benefits. State governments can increase access to a fertility preservation benefit by removing spousal criteria and expanding diagnostic criteria, and by including the benefit in Medicaid plans. What Gay and Bisexual Men Treated for Prostate Cancer Want in a Sexual Rehabilitation Program: Results of the Restore Needs Assessment
Conclusions Gay and bisexual men treated for prostate cancer want a recovery curriculum that explicitly addresses the sexual challenges they face before, during and after treatment. While differences were identified across race and treatment type, they were relatively few and minor in magnitude, suggesting that a single online curriculum could advance rehabilitation for this population. Prostate Multiparametric Magnetic Resonance Imaging Program Implementation and Impact: Initial Clinical Experience in a Community Based Health System
Conclusions While staging information with prostate magnetic resonance imaging was accurate early in the implementation of the program, lesion characterization improved with use of Prostate Imaging Reporting and Data System, version 2 criteria and standardized reporting. Regular multidisciplinary participation in community based prostate magnetic resonance imaging programs may maximize clinical impact. (Source: Urology Practice)

Patient Perceptions of Chaperones During Intimate Exams and Procedures in Urology Clinic
Conclusions A minority of patients preferred a chaperone during an intimate exam or procedure in urology clinic. Patients prioritized comfort level with the provider which trumped gender of provider, invasiveness of exam, and who the chaperone was. The use of chaperones during intimate exams and procedures is routine in many institutions. In an era of patient centered care, it is crucial to understand patient preferences and expectations. Laser Papillotomy for Chronic Flank Pain – A Reassessment of Efficacy in the Era of Automated Opioid Prescription Monitoring
Conclusions Laser papillotomy for chronic flank pain has a high failure rate, especially in patients who are on narcotic pain medication preoperatively. In the era of automated opioid prescription monitoring, urologists should use such information to make surgical decisions and properly counsel patients. (Source: Urology Practice)

The Role of Patient- and Procedure-Specific Factors on Urology Operating Room Peri-Operative Times
Conclusions Our analysis is a novel approach to assessing OR efficiency by characterizing the non-operative time a patient spends in the OR. Robotic cases have longer non-operative times and increasing patient complexity prolongs pre-operative time in the OR. Better preparation of complex patients pre-operatively will allow better use of constrained OR resources. (Source: Urology Practice)

Gender-based differences in discriminatory questions asked of urology applicants during residency interviews
Conclusions An alarming percentage of urology applicants are asked interview questions that violate employment law. Females are disproportionately questioned about age, parental status and intent for children. Education of interviewers regarding legally restricted questions is warranted. (Source: Urology Practice)
