Brainstem anaesthesia following single medial canthal peribulbar block: A case report and review
We report the first case of brainstem anaesthesia following a single medial canthal block in a patient scheduled for cataract surgery and explore the factors that set the stage for this serious complication. These include relatively short axial length of globe, needle trajectory, position of the needle tip in relation to the posterior plane of the globe and, the optic nerve. We introduce the concept of ‘Cornea to Canthus distance’ and urge that it is factored in when estimating the depth of needle tip during medial canthal peribulbar approach to avoid inadvertent retrobulbar intraconal injection. This case highlights t...

Publication date: Available online 16 April 2019Source: Trends in Anaesthesia and Critical CareAuthor(s): Daniela Godoroja, Massimiliano Sorbello, Michael MargarsonAbstractAirway management is often the major concern of anaesthetists when presented with a morbidly obese patient for general anaesthesia. All anaesthetists are encountering increasing numbers of these patients, presenting for all types of surgery. With the particular experience gained from bariatric surgery worldwide, there is now a greater evidence base to support and guide our practice. This review article aims to provide an update of the key points in safe a...

Flexible Laryngeal Mask with Pharyngeal Suction for Nasal Surgery
ConclusionsThe flexible laryngeal mask with pharyngeal suction catheters provided safe airway management for nasal surgeries without apparent glottic contamination or other airway-related complications and was not associated with an increased risk of postoperative sore throat in this retrospective observational study. A large controlled prospective study is needed to confirm these findings. (Source: Trends in Anaesthesia and Critical Care)

Between the devil and the deep red sea: Airway trauma during introducer-guided difficult intubation
Publication date: Available online 11 April 2019Source: Trends in Anaesthesia and Critical CareAuthor(s): Shreya Agarwal, Preeti GuptaAbstractA 46-year-old woman was scheduled for total thyroidectomy and had modified Mallampati class III airway. After anaesthesia was induced, laryngoscopy revealed Cormack–Lehane grade 3a view of the vocal cords with external laryngeal manipulation. A Portex single-use tracheal tube introducer was first introduced up to 27 cm mark, after eliciting the tracheal click and hold-up signs. A Portex tracheal tube size 7 mm ID was railroaded over the introducer with some difficulty because o...

Exploring the Impact of Overnight Call Stress on Anaesthesiology Senior Residents’ perceived Ability to Learn and Teach in an Asian Healthcare System: a Qualitative Study
Publication date: Available online 26 March 2019Source: Trends in Anaesthesia and Critical CareAuthor(s): Leonard Wei Wen Loh, John Song En Lee, Raymond Wee Lip GoyAbstractAcute stress can influence clinical performance, reasoning and decision-making. Our Anaesthesiology senior residents encounter physical and psychological stress during the overnight calls. To date, the impact of these stressors on their ability to learn and to supervise is unclear. We explored the nature of these stressors, and their influence on our trainees’ perceived learning and teaching. We conducted this qualitative study from August 2017 to Febru...
