Very Early Versus Early Percutaneous Coronary Intervention After Successful Fibrinolytic Therapy in Patients Scheduled for Pharmacoinvasive Strategy
ConclusionsNo-reflow phenomenon and the risk of recurrent ischemia is significantly lower in patients undergoing PCI very early after successful fibrinolytic therapy, but the risk of bleeding is increased in this time. So it is recommended that patients received successful fibrinolytic therapy to be subjected to very early PCI within 3 to 12 h from fibrinolysis. (Source: Global Heart)

Development and Validation of a Teaching Module for Echocardiographic Scoring of Rheumatic Mitral Stenosis
ConclusionsUse of this module has the potential to enhance the training of cardiologists in the echocardiographic assessment of mitral stenosis. Modified versions of this module or similar ones should be tested in targeted populations of cardiology trainees with the most exposure to mitral stenosis interventions. (Source: Global Heart)

The World Heart Federation Roadmap for Nonvalvular Atrial Fibrillation
ConclusionsAF affects millions of people worldwide and, left untreated, increases the risk and severity of stroke and heart failure. Although guidelines for the detection, diagnosis, and management of AF exist, there are gaps in implementation of these guidelines globally, and in particular in LMICs. This Roadmap identifies some potential solutions that may improve AF outcomes in LMICs but require further evaluation in these settings. (Source: Global Heart)

Feasibility of Using Mobile ECG Recording Technology to Detect Atrial Fibrillation in Low-Resource Settings
ConclusionsUsing mobile ECG technology in screening for AF in low-resource settings is feasible, and can detect a significant proportion of AF cases that will otherwise go undiagnosed. Further study is needed to examine the cost-effectiveness of this approach for detection of AF and its effect on reducing the risk of stroke in developing countries. (Source: Global Heart)

Is TAVR Ready for the Global Aging Population?
Publication date: December 2017Source: Global Heart, Volume 12, Issue 4Author(s): Travis Bergmann, Partho P. Sengupta, Jagat NarulaAbstractThe emergence of the global pandemic of chronic diseases necessitates critical assessment of interventions that can be targeted at both the individual and population levels. Among cardiovascular diseases, the increasing prevalence of valvular heart diseases such as aortic stenosis parallels the rising burden of atherosclerotic cardiovascular diseases. As an alternative to surgical aortic valve replacement, technological innovation has allowed development of minimally invasive transcathe...

Evaluating Health of Emerging Economies Through the Eyes of Heart Valve Disease in the Transcatheter Era
Publication date: December 2017Source: Global Heart, Volume 12, Issue 4Author(s): Sameer Arora, Cassandra J. Ramm, Amol A. Bahekar, John P. VavalleAbstractChina and India are the 2 fastest growing major world economies. However, they suffer from great differences in health policies, demographics, and rates of population growth. Whereas China has seen a steep decline in rheumatic heart disease (RHD) and a rise in life expectancy, India continues to suffer from a significant burden of RHD due to insufficient distribution of economic prosperity to health care, denser population, and ineffective application of World Health Org...

Secondary CV Prevention in South America in a Community Setting: The PURE Study
ConclusionsThere are large gaps in the use of proven medications for secondary prevention of cardiovascular disease in South America. Strategies to improve the sustained use of these medications will likely reduce cardiovascular disease burden substantially. (Source: Global Heart)

Time Trends in Lifestyle, Risk Factor Control, and Use of Evidence-Based Medications in Patients With Coronary Heart Disease in Europe: Results From 3 EUROASPIRE Surveys, 1999–2013
ConclusionsLifestyle habits have deteriorated over time with increases in obesity, central obesity, and diabetes and stagnating rates of persistent smoking. Although blood pressure and lipid management improved, they are still not optimally controlled and the use of evidence-based medications appears to have stalled apart from the increased use of high-intensity statins. These results underline the importance of offering coronary patients access to modern preventive cardiology programs. (Source: Global Heart)

Availability, Use, and Barriers to Cardiac Rehabilitation in LMIC
ConclusionsOur results illustrate that the published reports reflects heterogeneity of CR availability and use in LMIC. Overall, CR is insufficiently available and underutilized. Further characterization of CR in LMIC, especially in Asia and Africa, is necessary to develop targeted strategies to improve availability and utilization. Patient, physician, and systems factors must be addressed to overcome barriers to participation in CR in LMIC. (Source: Global Heart)

Relatives' Presence During Cardiopulmonary Resuscitation
ConclusionsAvoiding relatives' presence while performing cardiopulmonary resuscitation is the most frequent choice made by the surveyed physicians who treat critical Argentine patients. The main causes for discouraging family presence during cardiopulmonary resuscitation or other critical procedures include the following: risk of misinterpretation of the physician's actions and/or words; risk of a relative's decompensation; uncertainty about possible reactions; and interpretation of the relative's presence as negative.
