A Case for the Use of Validated Physiological Mortality Metrics to Guide Early Family Intervention in Intensive Care Unit Patients
In the current health care climate a large portion of health care dollars are spent in the final months of life, so ensuring that care provided is in line with the wishes of patients and their families is more critical than ever. On the one hand, surviving families often report that they wish they had been given prognostic information earlier and that, in retrospect, they would have made different treatment decisions if they had been given prognostic information. On the other hand, providers often are reluctant to discuss prognosis for various reasons, not the least of which is the inherently uncertain nature of prognostic...

Management of Blunt Pulmonary Injury
Thoracic injuries account for 25% of all civilian deaths. Blunt force injuries are a subset of thoracic injuries and include injuries of the tracheobronchial tree, pleural space, and lung parenchyma. Early identification of these injuries during initial assessment and resuscitation is essential to reduce associated morbidity and mortality rates. Management of airway injuries includes definitive airway control with identification and repair of tracheobronchial injuries. Management of pneumothorax and hemothorax includes pleural space drainage and control of ongoing hemorrhage, along with monitoring for complications such as...

Advanced Chest Imaging Interpretation of Acute Pulmonary Disorders
Bedside and advanced practice nurses in acute and critical care often view chest radiologic images of their patients. Correlation of findings on chest radiologic images with results of physical and other diagnostic assessments can provide information for making appropriate clinical judgments. Radiologic images of the chest available for acutely ill patients now include frontal/lateral chest radiographs and computed tomography (CT) scans. The purpose of this article is for the reader to review basic skills for interpreting chest radiographs, gain skill in reviewing CT scans of the chest, and be able to apply more advanced i...

Transferring Patients With Refractory Hypoxemia to a Regional Extracorporeal Membrane Oxygenation Center: Key Considerations for Clinicians
This article provides information essential for health care teams who refer their patients to such centers. The clinical indications for, and the use of, ECMO therapy in the management of refractory hypoxemia is briefly reviewed, followed by a description of how ECMO works to provide gas exchange and tissue perfusion. The primary considerations for circuit management, hemodynamic support, and pulmonary care are described, and significant complications of the therapy are identified. The remainder of the article focuses on the patient care and preparatory activities that occur before and during ECMO initiation, so that healt...

Development and Validation of a Clostridium Difficile Risk Assessment Tool
The incidence of Clostridium difficile infection has increased rapidly during the past decade, increasing lengths of stays in the hospital, costs, and mortality rates. To address this increased incidence, we performed a retrospective case-control study using known risk factors to develop a tool to determine which patients are at risk for infection. Multivariate analysis generated a combination of risk factors associated with development of infection including prior admission, endoscopy within 30 days, cephalosporin/fluoroquinolone use, length of stay 7 days or longer, age 65 years or older, body mass index less than 25, an...
