A Diagnostic Value of Serum Level Cyclase-Associated Plasma Protein 2 Versus Alpha Fetoproteins as a novel Biomarker for Detection of Hepato-Cellular Carcinoma in Egyptian Patients with Liver Cirrhosis
ConclusionCAP2 is significantly elevated in HCC group than in cirrhotic and control groups with better sensitivity and specificity than AFP at cut off values ≥15.9 ng/ml and ≥53.2 ng/ml respectively. Such results support using of CAP2 as a better diagnostic marker for HCC. AFP and CAP-2 were higher in multiple lesions than in single lesions, but the differences were significant only in CAP-2 with ≥27.3 ng/ml in differentiating multiple from s olitary lesions Considering the HCC if both of them were positive decreased sensitivity but had perfect specificity. Assessment of Doppler haemodynamic changes suggestive of portal hypertension in cirrhotic HCV patients after sustained virological response to direct antiviral agents
Conclusion: Doppler portal hypertensive parameters, as a marker of portal hypertension, were improved in parallel with the improvement in fibroscan values after viral clearance and its improvement in the current study mandate urgent treatment to avoid possible complications. (Source: QJM)

Correlation of Serum Vitamin D Levels to Vascular Access Dysfunction in Prevalent Hemodialysis
AbstractVascular access dysfunction in hemodialysis patients has been correlated to low serum vitamin D level in previous studies. Vitamin D deficiency and hepatitis C virus infection have been linked to endothelial cell dysfunction, promoting inflammatory cascade.30 negative -& 30 positive - hepatitis C virus patients on prevalent hemodialysis in Ain Shams University Hospitals, were enrolled in the study. All patients had access blood flow< 800  ml/min. For all patients we performed: complete physical examination, complete blood count, serum ferritin, ESR 1st and 2nd hours, CRP, blood urea, serum creatinine, seru...

Assessment of myostatin in obese males and females with and without fatty liver
ConclusionThis study revealed the association between BMI, gender and elevated myostatin at level of statistically significant value. The majority of the study population reported high range of myostatin were in about 82.5% the range was above the normal values. A large scale, multicentre study to provide strong evidence was recommended. (Source: QJM)

Evaluation of post-operative complications after Milligan- Morgan haemorrhoidectomy versus Stapled Haemorrhoidopexy techniques
ConclusionHaemorrhoidal disease is considered as one of the commonest ano-rectal diseases. Non-surgical measures whatever drug therapy or office procedures, can be presented to the patients with stage 1 and stage 2 haemorrhoids. However, when these measures fail, surgical intervention must be thought for the patients with stage 3 and stage 4 haemorrhoids. Surgical intervention should be tailored to each patient in accordance to the seriousness of symptoms, the magnitude of the external anorectal element and the presence of any concurrent ano-rectal morbidity. At present, there are many surgical interventions obtainable for...

Efficiency of Cruroplasty as a Treatment of Gastro-Esophageal Reflux Disease in Morbid Obese Patients with Hiatus Hernia during SleeveGastrectomy
ConclusionSG with HHR is feasible and safe, providing good management of GERD in obese patients with reflux symptoms. Small hiatal defects could be underdiagnosed at preoperative endoscopy and/or upper gastrointestinal contrast study. Thus, a careful examination of the crura is always recommended intraoperatively. Surgical Management of Early Hepatocellular Carcinoma
ConclusionLaparoscopic liver resection (LLR) is now considered a feasible alternative to open liver resection (OLR) in selected patients. Nevertheless studies comparing LLR and OLR are few and concerns remain about long-term oncological equivalence. LLR has short-term advantages and seemingly equivalent long-term outcomes and can be considered a feasible alternative to open surgery in experienced hands. (Source: QJM)

Comparative Study between Lichtenstein Repair and Trans Abdominal Pre Peritoneal Repair of Inguinal Hernia (TAPP)
ConclusionOur study showed that laparoscopic TAPP approach for inguinal hernia repair reduces early post-operative pain. Furthermore, it is related to less complications, although it takes a longer operative time and higher cost. (Source: QJM)

Rivaroxaban versus Warfarin in the Management of Deep Venous Thrombosis
ConclusionWe concluded in patients with unprovoked VTE, rivaroxaban has the potential to reduce both the risk of major bleeding and recurrent VTE compared to warfarin. And there ’s no need for adjusting the dose and INR monitoring, and Rivaroxaban has no food nor drug interactions. (Source: QJM)

Extreme Oncoplastic Breast Surgery in Patients Presented to Ain Shams University Hospital, Initial Experience
ConclusionThe combination of plastic surgery techniques with breast oncology surgery gives the surgeon a new tool for treatment of breast cancer. The preliminary results of our study indicate that selective patients (with multifocal/Multicenteric or large tumors) who were initially considered for mastectomy can be alternatively treated using Extreme oncoplasty (EO) followed by radiotherapy. EO can be considered as a safe and feasible surgical option for such patients without compromising aesthetic outcomes. (Source: QJM)

Evaluation of Serum Interleukin 36 in Egyptian Acne Vulgaris Patients versus Controls
ConclusionWe found that IL-36 is involved in acne pathogenesis and its serum levels are elevated even in mild cases with comedonal lesions suggesting the possible involvement in the early events of acne vulgaris. Further studies are required to demonstrate more detailed explanation of IL-36 pathogenic role in acne. (Source: QJM)
