Comparative Study between Automatic Tube Compensation (ATC) and Pressure Support Ventilation (PSV) as a Mode of Weaning from Mechanical Ventilation in Patients with Respiratory Failure
ConclusionIn ICU population, ATC was safe, reliable, and can be reasonably used for weaning trials. ATC confers a potential benefit in weaning duration, weaning category, number of Ss, failure of first SBT extubation outcome, ICU length of stay, complication, and mortality rate. In addition, ATC improves the predictive value of RR/TV and IWI in predicting weaning success. (Source: QJM)

Clinical and Laboratory Prognosis of Patients at Risk for Traumatic ARDS
ConclusionIdentifying potentially causal and modifiable factors that could lead to the development and testing of preventative ARDS therapies has been slow in part because of an incomplete understanding of which patients are likely to develop ARDS after major trauma. There are several ARDS predictors including an injury severity score (ISS), Acute Physiology and Chronic Health Evaluation (APACHE) II Score and others which try to identify trauma patients at greatest risk for ARDS. However, despite the intense research, only few effective therapies for ARDS have been postulated, including the lung protection strategies. (Source: QJM)

Red cell Distribution Width as a Prognostic Predictor in Ventilator Associated Pneumonia
ConclusionRDW is used as an inexpensive index that is routinely reported as a part of the complete blood count, RDW is an important marker for both diagnostic and prognostic purposes in all cause mortality in ICU patients not specifically in mechanically ventilated patients. Larger studies and clinical trials are recommended to determine the clinical outcome of RDW prognostic value in VAP. (Source: QJM)

Comparative Study between two Ultrasound Aided Methods in PreoperativeAssessment of Difficult Airway in Obese Patients
ConclusionUltrasonography can be a valuable adjunct in this aspect of airway assessment. Ultrasound assessment of pre-epiglottic tissue thickness at the level of the thyrohyoid membrane may be useful to predict restricted/difficult direct laryngoscopy and difficult intubation. The ratio of hyomental distance between neutral and extended positions may also be a good predictor of difficult direct laryngoscopy. (Source: QJM)

Single Intravenous Bolus versus Continuous Infusion of Tranexamic Acid to Reduce Blood Loss in Transurethral Resection of Prostate: a Prospective Randomized Double-Blind Study
ConclusionTranexamic acid causes more effective reduction in post-operative blood loss when used as a bolus followed by an infusion continued in the postoperative period in comparison to its use as a single intravenous bolus in transurethral resection of prostate. (Source: QJM)

Comparison between different weaning methods in chronic obstructive pulmonary disease patients with respiratory failure (BIPAP, CPAP and T piece)
ConclusionWe suggest that BIPAP improves patient gas exchange specially in hypercapnic patients, hemodynamics and associated with shortest duration of ICU stay and decrease rate of reintuabtion. (Source: QJM)

Effect of Hypercholesterolemia on Retina of Adult and Aged Male Albino Rat: A Histological and Immunohistochemical Study
ConclusionThe present work suggested that hypercholesterolemia is an important risk factor that can accelerates aging of healthy adult retina producing age related retinal degeneration. (Source: QJM)

Different Modalities of Management of Early and Late Traumatic Soft Tissue Defects of The Hand
ConclusionSoft tissue defects of the hand can be reconstructed effectively with multiple options that can restore function in addition to stable coverage. The use of grafts with several local and regional flaps have been identified including advancement flaps, rotation flaps in cases where local tissue is available and healthy. Which have the ability to match colour and texture of the recipient site, coverage of vital structures with minimal donor site morbidity. The preferred approach is the simplest method which can provide the most stable coverage. A careful selection of flap choice for coverage of a hand defects should...

The Assessment of Postoperative Patency and Flow Pattern of Twisted Vascular Pedicle in Propeller Perforator Flaps in Lower Extremities Reconstruction
ConclusionThe perforator propeller flaps are safe, reliable procedures and are considered as an ideal option in reconstructing small-medium defects of the middle and distal third of the leg providing similar skin texture with low rate of vascular obstruction. The present study documented patent vascular pedicle of propeller perforator flaps at three months postoperatively. (Source: QJM)

Surgical Modalities of Repair of Intra Articular Fractures of Proximal Interphalangeal Joint: A Meta-analysis
ConclusionThere is no consensus regarding the most effective technique for management of unstable PIPJ fractures. The extension block pinning, closed reduction and percutaneous pinning of the fracture fragments, dynamic distraction and external fixation, open reduction and internal fixation, and hemihamate reconstruction arthroplasty are safe and reliable and reduce the risk of iatrogenic damage when used to treat unstable dorsal fracture-dislocations of the PIP joint. A Prospective Clinical Study of the Effect of Recombinant Human Growth Hormone in Wound Healing of Severely Burned Children
ConclusionThe use of recombinant Growth hormone with a dose of 0.2  mg/Kg SQ 2 days per week with 3 days time interval in pediatric burn patients after their primary resuscitation from the burn injury, shows a marvelous improvement concerning the total body surface area of burn(TBSA) as the patient received the growth hormone showed a decrease total body surface area of burn(TBSA) than the control group. This may be accounted for the faster wound healing and readiness for grafting . (Source: QJM)

Impact of Topical Heparin on Healing of Skin Graft Donor Site in Burn Patients
ConclusionTopical heparin treatment, compared with topical conventional treatment, of the donor areas after skin grafting in burn patients resulted in better clinical outcome manifested as significantly shorter wound healing time and significantly less scarring with no treatment-related complications. Open Reduction and Internal Fixation Using Plate and Screws Versus Percutaneous Kirschner Wires Fixation for Second and Third Metacarpal Bones Fractures: A Meta-analysis
ConclusionBased on the reported results there is no evidence to suggest one fixation technique over another. The reported complications however for ORIF and K-wire fixation in the treatment of metacarpal fractures are unmistakably different for the two types of fixation. (Source: QJM)

Oncological Safety of Lipofilling in Patients with Breast Cancer: A Metaanalysis
ConclusionThrough this analysis, no evidence that lipofilling significantly increases the risk of locoregional recurrence when used as part of a breast reconstruction procedure after either mastectomy or BCS for breast cancer was found. Therefore, it can be considered an oncologically safe procedure in suitable patients with careful oncological follow-up. In the future, more longterm, adequately powered, multicentre controlled studies are needed. Additionally, more research should be carried out on the incidence of complications following lipofilling so that efforts can be made to prevent them and optimise fat graft survival. (Source: QJM)

Meta Analysis Study of Complications Following Repair of Mid Penile Hypospadius using Different Surgical Techniques
ConclusionThe Mathieu and Preputial island flap techniques for mid-penile hypospadias reconstruction are associated with a significantly lower risk for postoperative urethral stricture and may be the preferred method for hypospadias. However, the clinical implications of the results can be discussed because the risk of stricture in all procedures was low. The implications for research are obvious because there had been no studies that provided firm guidelines on the best method for the operative intervention for hypospadias. Further trials are still needed to confirm our findings. (Source: QJM)
