World War I and the Origins of Blood Transfusion for the Trauma Patient
Mil Med. 2023 Dec 22:usad483. doi: 10.1093/milmed/usad483. Online ahead of print.ABSTRACTThe use of blood transfusion to treat hemorrhagic shock is a relatively new treatment with its origins in World War I. Due to the severity and nature of injuries seen, World War I provided the stimulus to propel medicine forward and accept whole blood transfusion for the trauma patient.PMID:38140985 | DOI:10.1093/milmed/usad483 (Source: Military Medicine)

Telemedicine Improves Performance of a Two-Incision Lower Leg Fasciotomy by Combat Medics: A Randomized Controlled Trial
CONCLUSIONS: This randomized controlled trial shows that a HMD providing telemedicine support leads to significantly better performance of a two-incision lower leg fasciotomy by a combat medic with less iatrogenic muscle and venous damage.PMID:38141250 | DOI:10.1093/milmed/usad486 (Source: Military Medicine)

Maxillofacial Surgery in  Ukraine During a War: Challenges and Perspectives-A National Survey
CONCLUSIONS: According to this nationwide survey of Ukrainian maxillofacial surgeons during a year of the full-scale war, 86.6% of respondents were involved in the treatment of gunshot and ballistic injuries in civilians and combatants. The main problems reported by the respondents were (1) a lack of experience and knowledge related to the treatment of severe wounds, especially by secondary reconstruction, and (2) a deficit of resources (equipment, materials, and medications) under conditions of disrupted logistics and changes in the numbers and nosological distribution of patients. There were the opportunity to transfer t...

Leading Graduate Medical Education in the Face of Uncertainty
Mil Med. 2023 Dec 22:usad492. doi: 10.1093/milmed/usad492. Online ahead of print.ABSTRACTLeading through uncertainty poses significant challenges. The COVID-19 pandemic, the Afghanistan withdrawal, and the implementation of a new electronic medical records system led to massive shortages of health care workers throughout the Military Health System (MHS). This case describes the leadership challenges of a surgical residency program director during uncertain times.PMID:38140957 | DOI:10.1093/milmed/usad492 (Source: Military Medicine)

When Patient Portals Break Bad News
Mil Med. 2023 Dec 22:usad482. doi: 10.1093/milmed/usad482. Online ahead of print.ABSTRACTElectronic patient portals facilitate communication between providers and patients, but they can complicate the art of breaking bad news. Nearly half of oncology patients will view their diagnosis on a patient portal before speaking with a provider. Physicians and advanced practice providers receive training on how to deliver bad news in person and over the telephone. This is done in an empathetic and supportive manner. The patient's emotions are acknowledged, and questions are answered. While surveys of patients suggest that most peop...

Using Military Screening Questions to Anonymously Recruit Post-9/11 Era Service Members and  Veterans Using Online Survey Methods
CONCLUSIONS: Use of Qualtrics, Inc. to incorporate validation checks helped us to be more reasonably confident that we were collecting data from military service members who reported military sexual assault. While the probability of a person without current or past military service passing the validation checks was low, it was not impossible. An unanticipated benefit of this platform was the short duration of time it took to complete data collection; the sample was collected within about 6 weeks. This platform may be a good option for investigators who cannot collect face-to-face data.PMID:38140962 | DOI:10.1093/milmed/usa...

Strengthening Abortion Training: A Dilation and Evacuation Checklist in Military Resident Education
CONCLUSIONS: Use of an evidence-based checklist significantly improves resident knowledge and comfort with performing second trimester D&Es. In a post Dobbs environment, the military is an appropriate proxy for larger society and training programs need to develop alternatives and adjuncts to clinical training.PMID:38140981 | DOI:10.1093/milmed/usad491 (Source: Military Medicine)

Young Man With Primary Mucinous Cell Carcinoma of  the Eyelid: 36 Month Follow Up
We present a case of a 29-year-old male with an eyelid PCMC misdiagnosed as a chalazion. He underwent Mohs microscopic surgery for definitive treatment, with no recurrence at his 36 month follow up. Given the rarity and poorer prognosis in younger patients, a high index of suspicion is crucial for biopsying periocular lesions that fail conservative therapy.PMID:38140982 | DOI:10.1093/milmed/usad477 (Source: Military Medicine)
