Expanding Global Health Engagement through Multilateral Security Organizations
CONCLUSIONS: Although there is high demand for GHE, resourcing to enable implementation has not been prioritized. Therefore, multilateral organizations continue to support what is funded (e.g., disaster response) versus prioritizing capacity building or modifying authorities and appropriations to match demand. It is also worth noting most organizations included in this review support the European theater aligning to historical defense priorities, versus emerging threats in the Indo-Pacific region. Identifying a forum within these multilateral institutions to convene GHE policy makers and practitioners is a logical next ste...

Tube Thoracostomy Complications in  Patients With ARDS Requiring ECMO: Worse in COVID-19 Patients?
CONCLUSIONS: Tube thoracostomy placement in COVID-19 patients with ARDS requiring V-V ECMO is common, as are complications following initial placement. Clinicians should anticipate the need for re-intervention in this patient population. Small-bore (14Fr and smaller) pigtail catheters appeared to be safe and efficacious in this setting, but further study on tube thoracostomy management in ECMO patients is needed.PMID:38079460 | DOI:10.1093/milmed/usad454 (Source: Military Medicine)

Use of  Instant Messaging Applications by General Practitioners During Overseas Deployment: A Survey of the French Military Health Service
CONCLUSION: MAs were extensively utilized communication tools among GPs during their overseas deployments. Although the use of these applications seems essential in telemedicine, it raises several legal and ethical questions. Thus, we recommend employing these tools while ensuring medical and military confidentiality.PMID:38079462 | DOI:10.1093/milmed/usad463 (Source: Military Medicine)

Extending Air Force Physical Fitness Postpartum Dwell Time Improves Maternal Physical Readiness
CONCLUSIONS: The decision of the Air Force to increase the minimal required time between childbirth and postpartum physical fitness testing for women has positively impacted fitness test failure rates and anthropometric measure changes. Additional policy should be implemented to support postpartum holistic health interventions.PMID:38079464 | DOI:10.1093/milmed/usad456 (Source: Military Medicine)

A Descriptive Analysis of  Blood Products Utilized by a Forward Deployed Surgical Team in Syria
CONCLUSIONS: This analysis describes blood usage associated with one high volume forward deployed operative team and demonstrates the vital importance of the R2 split FST to provide coalition forces with surgical care in proximity to the point of injury. Over time, the supply chain has improved with more component therapy available at R2s; however, the need for walking blood bank and innovative solutions to care for all casualties must be part of small team capabilities. Liquid plasma use should be expanded as soon as it is feasible.PMID:38079465 | DOI:10.1093/milmed/usad459 (Source: Military Medicine)

Comparing Capabilities of  Simulation Modalities for Training Combat Casualty Care: Perspectives of Combat Medics
CONCLUSIONS: The study furthers our understanding of simulation modalities for medical training by providing insight from combat medics on the benefits, limitations, and considerations for implementing different modalities depending on the training context. These results may be helpful to instructors in selecting modalities for their programs.PMID:38079470 | DOI:10.1093/milmed/usad460 (Source: Military Medicine)
