Impact of female obesity and assisted reproduction on uncomplicated pregnancies and healthy births: a study of 428  336 births in Flanders
We examined the association of BMI and MAR with the following composite primary outcomes: ‘uncomplicated pregnancy and birth’ and ‘healthy baby’. We conducted Poisson regression and adjus ted for maternal age, parity, gestational weight gain, smoking and previous caesarean section.MAIN RESULTS AND THE ROLE OF CHANCEIn our study, 36.80% (n  = 157 623) of women had an uncomplicated pregnancy and birth according to the definition used. The predicted probability of having an uncomplicated pregnancy and birth for women with a BMI of 25 kg/m2 who conceived spontaneously was 0.33 (0.32 to 0.35), while it was 0.28 ...

International Committee for Monitoring Assisted Reproductive Technologies world report: assisted reproductive technology, 2014 †
This report covers approximately two-thirds of world ART activity. China is a major contributor of global cycles missing from this report. Continued efforts to improve the quality and consistency of ART data reported by registries are still needed, including the use of internationally agreed standard definitions (The International Glossary of Infertility and Fertility Care). A new method was introduced in this report to calculate ART utilization and number of babies born following ART; therefore, these results are not directly comparable with previous reports.WIDER IMPLICATIONS OF THE FINDINGSThe ICMART world reports provi...

What is the threshold of mature oocytes to obtain at least one healthy transferable cleavage-stage embryo after preimplantation genetic testing for fragile X syndrome?
This study is retrospective, analyzing a limited number of cycles. Moreover, the patients who were included in a fresh PGT cycle were selected on ovarian reserve parameters and show high values in ovarian reserve tests. This information could influence our conclusion.WIDER IMPLICATIONS OF THE FINDINGSThe results relate only to the target population of this study, with a correct ovarian reserve of AMH>1 and AFC>10. However, the information provided herein extends knowledge about the current state of COS forFMR1 mutation carriers in order to provide patients with proper counseling regarding the optimal number of oocyte...

Liquid biopsy: state of reproductive medicine and beyond
ABSTRACTLiquid biopsy is the process of sampling and analyzing body fluids, which enables non-invasive monitoring of complex biological systemsin vivo. Liquid biopsy has myriad applications in health and disease as a wide variety of components, ranging from circulating cells to cell-free nucleic acid molecules, can be analyzed. Here, we review different components of liquid biopsy, survey state-of-the-art, non-invasive methods for detecting those components, demonstrate their clinical applications and discuss ethical considerations. Furthermore, we emphasize the importance of artificial intelligence in analyzing liquid bio...

Assessing couples ’ preferences for fresh or frozen embryo transfer: a discrete choice experiment
AbstractSTUDY QUESTIONWhat are couples ’ preferences for fresh embryo transfer versus freezing of all embryos followed by frozen embryo transfer and the associated clinical outcomes that may differentiate them?SUMMARY ANSWERCouples ’ preferences are driven by anticipated chances of live birth, miscarriage, neonatal complications, and costs but not by the differences in the treatment process (including delay of embryo transfer linked to frozen embryo transfer and risk of ovarian hyperstimulation syndrome (OHSS) associated wit h fresh embryo transfer).WHAT IS KNOWN ALREADYA policy of freezing all embryos followed by tran...

Optimisation of the timing of fertilisation assessment for oocytes cultured in standard incubation: lessons learnt from time-lapse imaging of 78 348 embryos
AbstractSTUDY QUESTIONUsing time-lapse data, can the current consensus for the timing of fertilisation assessment of oocytes, cultured in standard incubation, be optimised?SUMMARY ANSWERThe optimum time to perform fertilisation assessment for oocytes cultured in standard incubation is 16.5  ± 0.5 h post-insemination (hpi), and the current consensus requires modification in order to minimise the chance of fertilisation being missed.WHAT IS KNOWN ALREADYTime-lapse incubation allows the embryologist to retrospectively review collated images of oocytes and embryos to capture important embryological observations that may...

Comparing endometriotic lesions with eutopic endometrium: time to shift focus?
This article explores the histological features of endometriotic lesions, highlighting their sometimes underappreciated heterogeneity. We note the variability in evidence for and against the menstrual cycle responsiveness of lesions and consider the utility of drawing parallels between endometriotic lesions and eutopic endometrium. We ask whether histopathologic features beyond just the presence/absence of endometrial-like glands and/or stroma could help improve disease stratification. At the same time, we acknowledge the desire of many clinicians and patients to avoid invasive surgery thereby limiting the ability to histo...

Successful pregnancy after ovulation induction with human chorionic gonadotropin in a woman with selective luteinising hormone deficiency
ABSTRACTSelective LH deficiency has been described in several men, but only in two women who presented normal pubertal development but secondary amenorrhoea due to anovulation. Despite its rarity, this condition represents a valuable model for studying the processes regulated by FSH or LH during late folliculogenesis and ovulation in humans. A woman previously diagnosed with selective LH deficiency due to a homozygous germline splice site mutation inLHB (IVS2  + 1G→C mutation) was submitted to an individualised ovarian induction protocol, first with recombinant LH and then with highly purified urinary hCG. Ovarian fo...

Genetic diagnosis, sperm phenotype and ICSI outcome in case of severe asthenozoospermia with multiple morphological abnormalities of the flagellum
AbstractSTUDY QUESTIONAre ICSI outcomes impaired in cases of severe asthenozoospermia with multiple morphological abnormalities of the flagellum (MMAF phenotype)?SUMMARY ANSWERDespite occasional technical difficulties, ICSI outcomes for couples with MMAF do not differ from those of other couples requiring ICSI, irrespective of the genetic defect.WHAT IS KNOWN ALREADYSevere asthenozoospermia, especially when associated with the MMAF phenotype, results in male infertility. Recent findings have confirmed that a genetic aetiology is frequently responsible for this phenotype. In such situations, pregnancies can be achieved usin...
