Late outcomes of ST-elevation myocardial infarction treated by pharmaco-invasive or primary percutaneous coronary intervention
ConclusionPatients who underwent late pPCI had higher mortality rates than those undergoing a pharmaco-invasive strategy. Despite rescue PCI being required in a third of patients, a pharmaco-invasive approach should be considered when delays to PCI are anticipated, as it achieves better outcomes than late pPCI. Long-term mortality, cardiovascular events, and bleeding in stable patients 1 year after myocardial infarction: a Danish nationwide study
ConclusionsFor patients who were event-free 1 year after MI, the long-term risks of mortality, cardiovascular events, and bleeding decreased significantly, along with an improved use of guideline-recommended treatments between 2000 and 2017. In the most recent period, 1 year after MI, the risk of additional events was lower than previously reported. (Source: European Heart Journal)

Serial troponin-T and long-term outcomes in suspected acute coronary syndrome
ConclusionsIndividuals with suspected acute coronary syndrome and two consecutively elevated hsTnT concentrations consistently had the highest risk of death. Mortality was very low in subjects with two normal hsTnT concentrations, irrespective of changes between measurements. (Source: European Heart Journal)

High-risk calcified plaque preoperatively detected by non-contrast T1-weighted magnetic resonance imaging and electrocardiogram-gated computed tomography
A 76-year-old man with chronic kidney disease presented with worsening angina pectoris. Coronary angiography (CAG) revealed an ischaemia-inducible stenosis in the distal left main trunk (LMT), continuing to the proximal left anterior descending artery (Panel A). Non-contrast electrocardiogram (ECG)-gated computed tomography (CT) showed a heavily calcified plaque with a low-intensity area surrounded by high-intensity signals (dark-crescent sign) in the distal LMT; non-contrast T1-weighted magnetic resonance imaging (T1WI) revealed the presence of a high-intensity plaque (HIP) (Panels B –D). Intravascular ultrasound (Panel...

Maternal and neonatal complications in women with congenital heart disease: a nationwide analysis
ConclusionsThis population-based study illustrates a reassuringly low maternal mortality rate in a highly developed healthcare system. Nevertheless, maternal morbidity and neonatal morbidity/mortality were significantly increased in women with ACHD and their offspring compared to non-ACHD controls highlighting the need of specialized care and pre-pregnancy counselling. The ‘10 commandments’ for the 2021 ESC guidelines on cardiac pacing and cardiac resynchronization therapy
The guidelines on cardiac pacing and cardiac resynchronization therapy provide an updated overview on indications for pacing, peri-operative management, and postoperative follow-up. A special emphasis is given to patient-centred care and shared decision-making. The following 10 commandments summarize main points. (Source: European Heart Journal)
