Joint exposure to various ambient air pollutants and incident heart failure: a prospective analysis in UK Biobank
Conclusion  Our results indicate that the long-term joint exposure to various air pollutants including PM2.5, PM10, PM2.5 –10, NO2, and NOx is associated with an elevated risk of incident HF in an additive manner. Our findings highlight the importance to comprehensively assess various air pollutants in relation to the HF risk. (Source: European Heart Journal)

Cardiology in 2021
‘May you live in interesting times’ is an old Chinese proverb (or a curse.) In 2021, a large number of societal, economic, and political changes are altering healthcare systems around the world. These and the widespread use of digital technology are having a profound effect on all areas of healt h care, including cardiology. The traditional physician–patient relationships are altered as a result, in part, because most patients in industrialized nations have access to the Internet with its wealth of information, much of which is incomplete and some is inaccurate. The popular press and soci al media provide rankings of...

Silent brain infarcts and early cognitive outcomes after transcatheter aortic valve implantation: a systematic review and meta-analysis
ConclusionsSilent brain infarcts are common after TAVI; and diabetes, kidney disease, and pre-dilation increase overall SBI risk. While higher numbers of new SBIs appear to adversely affect early neurocognitive outcomes, long-term follow-up studies remain necessary as TAVI expands to low-risk patient populations. The use of CEPD did not result in a significant decrease in the occurrence of SBI. (Source: European Heart Journal)

The REDUCE-IT verdict on eicosapentaenoic acid and cardiovascular outcome challenged with STRENGTH
Key pointsSTRENGTH,1 an industry-sponsored, double-blind, randomized, controlled trial investigated the effects of a carboxylic acid (CA) formulation of eicosapentaenoic acid (EPA) and docosahexaenoic acid (DHA) (omega-3 CA) vs. corn oil in 13 078 participants with high cardiovascular (CV) risk, hypertriglyceridaemia, and low levels of high-density lipoprotein cholesterol.Eligible patients were randomized to receive 4 g/day of omega-3 CA or corn oil in addition to standard preventive therapies, including statins (high-intensity in 50%), renin-angiotensin-aldosterone system (RAAS) blockers (in 81%), and antiplatelet agents ...

Death of a child and the risk of atrial fibrillation: a nationwide cohort study in Sweden
ConclusionsDeath of a child was associated with a modestly increased risk of AF. Our finding that an increased risk was observed also after loss of a child due to unnatural deaths suggests that stress-related mechanisms may also be implicated in the development of AF. Patient-tailored antithrombotic therapy following percutaneous coronary intervention
AbstractDual antiplatelet therapy has long been the standard of care in preventing coronary and cerebrovascular thrombotic events in patients with chronic coronary syndrome and acute coronary syndrome undergoing percutaneous coronary intervention, but choosing the optimal treatment duration and composition has become a major challenge. Numerous studies have shown that certain patients benefit from either shortened or extended treatment duration. Furthermore, trials evaluating novel antithrombotic strategies, such as P2Y12 inhibitor monotherapy, low-dose factor Xa inhibitors on top of antiplatelet therapy, and platelet func...

European position paper on the management of patients with patent foramen ovale. Part II - Decompression sickness, migraine, arterial deoxygenation syndromes and select high-risk clinical conditions
AbstractPatent foramen ovale (PFO) is implicated in the pathogenesis of a number of medical conditions but to date only one official position paper related to left circulation thromboembolism has been published. This interdisciplinary paper, prepared with the involvement of eight European scientific societies, reviews the available evidence and proposes a rationale for decision making for other PFO-related clinical conditions. In order to guarantee a strict evidence-based process, we used a modified grading of recommendations, assessment, development, and evaluation (GRADE) methodology. A critical qualitative and quantitat...
