β-blockers are not all the same: pharmacologic similarities and differences, potential combinations and clinical implications
Curr Med Res Opin. 2024;40(sup1):15-23. doi: 10.1080/03007995.2024.2318058. Epub 2024 Apr 10.ABSTRACTβ-blockers are a heterogeneous class, with individual agents distinguished by selectivity for β1- vs. β2- and α-adrenoceptors, presence or absence of partial agonist activity at one of more β-receptor subtype, presence or absence of additional vasodilatory properties, and lipophilicity, which determines the ease of entry the drug into the central nervous system. Cardioselectivity (β1-adrenoceptor selectivity) helps to reduce the potential for adverse effects mediated by blockade of β2-adrenoceptors outside the myocar...

The current position of β-blockers in hypertension: guidelines and clinical practice
Curr Med Res Opin. 2024;40(sup1):25-32. doi: 10.1080/03007995.2024.2318003. Epub 2024 Apr 10.ABSTRACTThe benefits of improved clinical outcomes through blood pressure (BP) reduction have been proven in multiple clinical trials and meta-analyses. The new (2023) guideline from the European Society of Hypertension (ESH) includes β-blockers within five main classes of antihypertensive agents suitable for initiation of antihypertensive pharmacotherapy and for combination with other antihypertensive agents. This is in contrast to the 2018 edition of ESH guidelines that recommended β-blockers for use primarily in patients with ...

Sympathetic overactivity, hypertension and cardiovascular disease: state of the art
Curr Med Res Opin. 2024;40(sup1):5-13. doi: 10.1080/03007995.2024.2305248. Epub 2024 Apr 10.ABSTRACTCardiovascular disease (CVD) remains the most prevalent cause of premature death worldwide. It had been suspected for decades that increased activity of the sympathetic nervous system (SNS) might play a pathogenetic role in the development and progression of hypertension, heart failure (HF) and CVD. The use of microneurographic techniques to directly assess the SNS has allowed this field to advance considerably in recent years. We now have compelling evidence for a key role of sympathetic overactivity in the pathogenesis and...

Heart failure management with β-blockers: can we do better?
Curr Med Res Opin. 2024;40(sup1):43-54. doi: 10.1080/03007995.2024.2318002. Epub 2024 Apr 10.ABSTRACTHeart failure (HF) is associated with disabling symptoms, poor quality of life, and a poor prognosis with substantial excess mortality in the years following diagnosis. Overactivation of the sympathetic nervous system is a key feature of the pathophysiology of HF and is an important driver of the process of adverse remodelling of the left ventricular wall that contributes to cardiac failure. Drugs which suppress the activity of the renin-angiotensin-aldosterone system, including β-blockers, are foundation therapies for the...

Risk factors for recurrence in patients with hormone receptor-positive/human epidermal growth factor receptor 2-negative early breast cancer in Japan: a systematic literature review and meta-analysis
CONCLUSIONS: Identified risk factors were consistent with the previous reports, and this study provides quantitative summary of risk factors for HR+/HER2- early breast cancer recurrence in Japan. (PROSPERO Registration ID, CRD42022338391.).PMID:38597173 | DOI:10.1080/03007995.2024.2332436 (Source: Current Medical Research and Opinion)

Real-world clinical outcomes and healthcare costs in patients with Crohn's disease treated with vedolizumab versus ustekinumab in the United States
CONCLUSIONS: In this real-world study, similar treatment persistence, dose escalation, and rates of opportunistic or serious infections were observed with VDZ- and UST-treated patients with CD. However, VDZ was associated with a significantly lower cost outlay for healthcare systems.PMID:38586979 | DOI:10.1080/03007995.2024.2326585 (Source: Current Medical Research and Opinion)

Pregnancy and fetal outcomes following paternal exposure to glatiramer acetate
CONCLUSIONS: This study did not indicate an increase in the rate of adverse pregnancy and fetal outcomes after paternal exposure to GA. These results provide additional information regarding pregnancy outcomes following paternal exposure to GA for healthcare professionals, male patients and their female partners who are considering pregnancy while their male partner is using GA.PMID:38577712 | DOI:10.1080/03007995.2024.2337669 (Source: Current Medical Research and Opinion)

Real-world effectiveness of long-acting injectable vs. oral antipsychotics in patients with bipolar I disorder: a 1-year retrospective observational study
CONCLUSIONS: In real-world data, there is no evidence that LAI antipsychotics (compared to OAP) are superior in the maintenance treatment of BD. These results are important in terms of reflecting clinical practices for the treatment of BD-I. These results do not devalue the use of LAI therapy in BD; however, more studies are needed to identify positive predictors for LAI treatments in BD.PMID:38557295 | DOI:10.1080/03007995.2024.2337685 (Source: Current Medical Research and Opinion)

Skin disorders in women with poor obstetric history: < em > MTHFR < /em > polymorphisms and importance of preconceptional counseling
CONCLUSIONS: The increased prevalence of psoriasis and acrochordons among women with MTHFR deficiency underscores the complex relationship between genetic factors and dermatological health. Our findings emphasized the critical role of MTHFR polymorphisms not only in poor obstetric history but also as significant contributors to skin disorders. This dual association highlights the importance of comprehensive preconception counseling, especially customized for women affected by skin disorders.PMID:38557333 | DOI:10.1080/03007995.2024.2337668 (Source: Current Medical Research and Opinion)
