Bilateral indirect ovarian inguinal hernia in a young female with type 1 Mayer –Rokitansky–Küster–Hauser syndrome: An extremely rare clinical context
Key Clinical MessageIncidence of bilateral inguinal hernia encompassing bilateral ovaries in adult female is very thin and concomitant association with Mayer –Rokitansky–Küster–Hauser syndrome is out of ordinary. Along with surgical management of hernia, these females need multidisciplinary slant to manage gynecological, social, and emotional issues.AbstractIn mature females, bilateral ovarian inguinal hernias are a rarity. In this situation, ultrasonography is the basic adjunct to confirm the diagnosis. Mayer –Rokitansky–Küster–Hauser syndrome is typically linked to ovarian hernias in grown-up females. The m...

Durable complete response to PET ‐CT driven stereotactic radiation therapy plus pembrolizumab for pleomorphic Pancoast cancer: Case report and literature review
Key Clinical MessagePET-driven SBRT plus pembrolizumab as first-line therapy against pleomorphic Pancoast cancer appears beneficial, probably due to high equivalent doses of SBRT on photopenic necrotic core and synergic immune system stimulation of immunoradiotherapy. Folliculotropic mycosis fungoides associated with follicular mucinosis: A case report and mini review
Histological features of follicular mycosis fungoides. (A-B) Dense superficial and deep dermal lymphocytic infiltrate with a predominantly perifollicular/folliculocentric distribution extending into the mid-dermis. Atypical lymphocytes are located along the basal cell layer of the hair follicle epithelium. Some neoplastic lymphocytes are surrounded by a halo. Although there is epidermal thinning, there is no atypia or epidermotropism, or Pautrier microabscesses. The interfollicular epidermis is not involved. (Hematoxylin and eosin staining, original magnification, A:x20, B:x40). (C-D) Exocytosis of small and medium-sized h...

Evaluation of flow dynamics in distal stent graft ‐induced new entry using 4D flow MRI
Key Clinical MessageDistal stent graft-induced new entry may occur after stent grafting for aortic dissection. Four-dimensional magnetic resonance imaging is useful for predicting outcomes, showing accelerated flow and increased wall shear stress, indicating further false lumen expansion. Sudan black poisoning resulted in methemoglobinemia in a baby with congenital chyloperitoneum
Key Clinical MessageTreatment of congenital chyloperitoneum is a challenge. Conservative methods may be ineffective. Preoperative visualization of the site of lymphatic leakage is crucial, but radiological imaging is technically complicated and may not provide sufficient information, especially in small patients. To ease the detection of lymphatic leakage during surgery, preoperative feeding with fat-rich formula with Sudan Black has been recommended. However, administration of Sudan Black may result in life-threatening methemoglobinemia and liver damage without any advantage of revealing leakage during surgery. We recomme...

Rare co ‐occurrence of tonsillar follicular dendritic cell sarcoma and schizophrenia: A comprehensive study
This study investigated the infrequent occurrence of tonsillar follicular dendritic cell sarcoma (FDCS) co-existing with schizophrenia, presenting a comprehensive examination of clinical, pathological, and literature aspects. A systematic literature review was conducted, focusing on articles related to “schizophrenia” and “sarcoma,” with in-depth analysis of included case reports. Clinical data, pathological findings, and patient follow-up information were collected and synthesized. The study detailed a rare case of FDCS in the tonsil concurrent with schizophrenia, providing insights into diagnosis, treatment, and ...

Consanguineous couple with SDHD gene mutations: Diagnosis, treatment, and implications of family genetic testing
DOTATOC-PET/CT of both spouses; the upper one showing no evidence of a cervico-thoraco-abdominal lesion suspicious for malignancy (wife), the lower one showing a stable glomus tumor on the right side of the neck (husband). Key Clinical MessagesThe newly published clinical consensus guideline on the management of PGL/PCC is helpful for decision-making for diagnostics and treatment. Still, the treatment of patients with SDHD gene mutations requires an individual approach and those patients belong to multiprofessional teams. It is often assumed that spouses are genetically unrelated. However, the genetic relationships between...

A rare case of Gardner syndrome in an African adult male: A case report
Key Clinical MessageGardner's syndrome with the complete manifestation of colonic and extracolonic features is uncommon. Therefore, every clinician should view extracolonic features with a high index of suspicion. This may be key to early diagnosis, definitive management in these patients and importantly, helps prevent malignant transformation of existing colonic polyps. (Source: Clinical Case Reports)

Iron deficiency ‐induced thrombocytosis presenting with blue toe syndrome
We report the case of a 38-year-old Saudi male who presented with blue toe syndrome and laboratory investigations revealed severe thrombocytosis secondary to iron deficiency. The patient was managed with analgesics, antiplatelets, anticoagulation, intravenous fluids, and iron supplementation. Subsequently, his symptoms resolved within a few days. IDIT is crucial to consider as a possible cause of microvascular thrombosis disorders, especially in patients with severe thrombocytosis. (Source: Clinical Case Reports)

Fabrication of a mandibular implant ‐retained overdenture using an existing subperiosteal implant: A clinical report
Key Clinical MessageA 64-year-old edentulous woman with a mandibular fracture received a subperiosteal implant for fracture fixation and dental rehabilitation. However, the ball abutments were submerged by the soft tissue because they were too short. Therefore, we designed a connector to lengthen the attachment and achieve adequate stability and retention for the overdenture. (Source: Clinical Case Reports)

Case report: Emerging BRCA mutation confers benefit from olaparib after chemotherapy intolerance in advanced triple ‐negative breast cancer
Key Clinical MessageIn a patient with metastatic breast cancer, an acquired BRCA mutation in the BRCA gene was detected, resulting in benefits from olaparib treatment. This underscores the importance of ongoing genetic phenotype testing after paclitaxel chemotherapy.AbstractTriple-negative breast cancer (TNBC) is associated with a poor prognosis and elevated mortality risk. BRCA mutations are commonly regarded as prevalent mutations in TNBC patients, strongly associated with congenital familial heredity. Dynamic changes in mutation sites, however, are rarely reported. In this case report, we report a 59-year-old TNBC patie...

A case report on multisystem inflammatory syndrome in children (MIS ‐C) associated with COVID‐19 infection
We report a case involving a fourteen-year-old male with COVID-19 infection who developed multisystem inflammatory disease. A previously healthy child presented with a history of 10  days of fever and cough, along with diarrhea, and vomiting for 3 days. His COVID-19 infection was confirmed through Polymerase Chain Reaction (PCR), and the laboratory values were remarkable for high levels of C-reactive protein, D-dimers, B-type natriuretic peptide (BNP), and troponin I. He de veloped circulatory shock on the second day of the presentation and needed inotropic support. Steroids and intravenous immunoglobulin (IVIG) were s...

Antimicrobial ‐associated encephalopathy due to ampicillin
Key Clinical MessageBecause the β-lactam ring has a molecular structure similar to that of gamma-aminobutyric acid (GABA) neurotransmitters, it binds to GABA A receptors and inhibits GABAergic transmission, causing AAE. The possibility of antimicrobial-associated encephalopathy should be considered in cases of neurological or psy chiatric symptoms after initiating an antimicrobial regimen.
