Recurrence of DVT while on newer anticoagulant therapy ( Rivoroxaban 20mg once daily).

Help please
Hy everybody, I'm a young french cardiologist and I want to come in UK in 1 or 2 years. I have two questions 1) Is it difficult for a french cardiologist to find a job in London ? (NHS system or private) 2) During this 1 or 2 next year again in France, I want to learn a technique and I hesitate with cardiovascular imaging (Coronary TDM and Cardiac MRI) And Pacing (single/dual chamber pacemaker or defibrillator). Which of this do you advise me to have the best "profile" to come to work in UK ? A 76 years old  man, type 2 diabetic for 10 years, on 1.0 gram of Metformin, HBA1C of 8%, BMI of 28.5, No Microalbuminuria, small right carotid intima thickness but no plaques. He has a healthy Life Style with regular exercises and Diet, taking also 10 mg Atorvastatin, and on Amlodipine 5 mg/day had a 24 hour  Arterial Blood Pressure Monitoring requested by his GP because of his Hypertension. ssri + varfarin
Hello to everyone! It is known from recent researche that ssri antidepressants increase bleeding tendency. Do you consider smoking status when prescribing clopidogrel?
Clopidogrel was a massive earner for its manufacturer while its patent lasted, but like so many drugs which have cost health systems billions in long-term treatment, it may not actually do that much for most people who take it. A meta-analysis of nine trials in patients with established cardiovascular disease, the effect of clopidogrel was highly dependent on smoking status. Those who smoked showed a 25% reduction in a composite outcome of cardiovascular death, myocardial infarction or stroke, compared with a reduction of 8% in non-smokers. Is heart failure the most lethal complication of diabetes?
I've been following a debate at the European Diabetes meeting on this point. The argument is that diabetologists and cardiologists fail to notice that heart failure is the most adverse outcome of diabetes and is killing more patients than we seem to notice. Is this right?
