Decompensation in Patients With Nonalcoholic Steatohepatitis: A Multihit Hypothesis

We have read with great interest the article by Bassegoda et  al1 about the increased prevalence of clinical decompensation in patients with nonalcoholic fatty liver disease (NAFLD) at lower thresholds of hepatic venous pressure gradient (HVPG) as compared with those with viral disease. The present study suggests an important concept about underestimation of portal pressures by measuring HVPG in patients with NAFLD when compared with viral etiology. Although not specifically evaluated, authors suggested an existence of presinusoidal component as a possibility of this underestimation of portal pressures.
