Primary sacral lymphoma with multiple bone involvement evaluated using 18F-FDG PET/CT

A 39-year-old man presented with left lower back pain, difficult defecation, left lower back tenderness, perineal numbness and renal dysfunction with elevated levels of serum creatinine and uric acid. Pelvic X-ray showed a patchy shadow in the left sacrum (Fig. 1a). Computed tomography (CT) demonstrated focal sacral osteolysis with soft tissue mass (Fig. 1b and c, arrows). Magnetic resonance imaging (MRI) revealed abnormal-signal lesions in the sacrum and right iliac ala (Fig. 1d –i, arrows). Chordoma with bone metastasis was suspected. Bone scintigraphy (Fig. 2a) showed increased tracer uptake in the sacrum and sacroiliac joint.
