Study protocol: CareAvenue program to improve unmet social risk factors and diabetes outcomes- A randomized controlled trial

In this study, we will test the effectiveness of CareAvenue – an automated e-health tool that screens for unmet social risk factors and informs and activates individuals to take steps to connect to resources and engage in self-care.
