New variation of median arcuate ligament compression causing hepatic arterial hypoperfusion during liver transplantation

We present a case where there was ligamentous compression of the coeliac artery at the origin of the splenic. The arterial flow after anastomosis was poor and division of the ligament immediately improved flow at the anastomosis. The patient had a normal post operative course. Awareness of the possibility of these rare anatomical distortions is essential in order to make the appropriate intraoperative decision for better outcome.
