First, I recommend implanting Ferrara-type ICRS and then explanting the toric ICL (two-step procedure). The implant of a spherical or toric IOL in cases of keratoconus should be considered for those cases where the morphology of the cornea has been previously corrected and stabilized by Ferrara-type ICRS.1
