ICDAS is a visual diagnostic system that is highly reproducible and accurate for coronal carious lesions detection, but only moderately reproducible and accurate for assessing lesion progression.

ConclusionsICDAS is a robust caries scoring system to assess the detection of coronal lesions in the permanent and primary dentitions related to both non-cavitated enamel and cavitated dentinal lesions. Active lesions progress more than non-active lesions, especially when they are classified as being cavitated. The reproducibility and accuracy of the additional activity assessments were moderate.
