Does prematurity unmask disease?
A 55 year old man with a history of asthma, chronic obstructive pulmonary disease, hypertension, diabetes, current smoking, and cocaine use presented to the emergency department with dyspnea on exertion, wheezing, and a dry cough for 4 days. On physical examination, his vital signs were normal, ches t examination revealed audible expiratory wheezing, and cardiovascular examination was normal. An electrocardiogram (ECG) was obtained (Fig. 1A). What is the diagnosis? Is any workup required?
