Facing obesity: Adapting the collaborative deliberation model to deal with a complex long-term problem

Raising the subject of obesity is fraught with difficulty. Clinical conversations are complicated by misperceptions about its causes and management, unrealistic expectations, weight stigma, lack of provider training, time and reimbursement constraints, and complex underlying emotional aspects [1 –8]. As a result, patients do not receive the care they need to improve health and prevent associated co-morbidities [5,9]. There is an emerging understanding that care plans need to be co-developed with patients based on an understanding of biopsychosocial processes that affect weight and health [3,4,7,10–13].
