Apical hypertrophy of left ventricle is a relatively rare manifestation of hypertrophic cardiomyopathy, sometimes associated with coronary fistulae. We show the case of a coronary microfistulae draining on left ventricle associated with an apical hypertrophic cardiomyopathy which manifested for the first time as an acute coronary syndrome NSTEMI.
