Empowering the Aging with Mobile Health: A mHealth Framework for Supporting Sustainable Healthy Lifestyle Behavior

In the rapid evolution of healthcare, patients continue to ask providers to shift from volume-based care (i.e., fee for service) to a value-based reimbursement structure (i.e., fee for value), which is a “population health” approach.1 However, for healthcare providers to shift to such a model, a clear understanding of the relationship between population health and healthy lifestyle management (HLM) is required. Such an approach not only benefits the patient, but incentivizes providers and health care services to deliver quality care (at the lowest cost)—while providing patients a lifestyle management approach with the greatest potential to improve their personal health.
