Clinical pearls in gastroenterology

A 58-year-old male is referred to hepatology clinic for worsening of liver enzymes after initiation of atorvastatin 40  mg once daily. The patient has metabolic syndrome with obesity (BMI 33 kg/m2), type 2 diabetes controlled on oral agents, hypertension and dyslipidemia. He was recently hospitalized for angina and has been treated medically. Since discharge he has been well without angina. He denies any muscle aches.
